We are seeking a Substance Use Disorder Services (SUDS) Treatment Coordinator to join our team.  The purpose of this position is to provide assistance to clients and providers in the coordination of care. This may include resource and referral coordinating, information gathering, and data reporting.

Essential Functions include:

  • Assistance in scheduling SUDS clients and making referrals based upon treatment needs.
  • Assistance in coordination with and follow up for services as identified in the treatment plan
  • Facilitation of referrals to substance use disorder services as indicated by a client’s medical provider, comprehensive assessment, or treatment plan
  • Assistance with referrals to economic assistance, social services, housing resources, and prenatal care according to the client’s needs
  • Life skills advocacy and support accessing treatment follow-up, disease management, and education services, including referral and linkage to long-term services and supports needed
  • Provide one on one follow up with identified treatment needs per the treatment team
  • Collaborate with community hospital and coordinate withdrawal management
  • Provide one on one EIMR skills, educational, and/ or resources with clients
  • Participate in county meetings with SUDS team to collaborate with probation officers, case managers, etc.

Qualifications include:

  • Bachelor’s degree in one of the behavioral sciences or related fields or current certification as and alcohol and drug counselor
  • At least one year of supervised experience working with clients with substance use disorder 
  • Must be skilled in the process of identifying and assessing a wide range of client needs
  • Knowledgeable about local community resources and how to use those resources for the benefit for the client
