North Suffolk is looking for a Community Coach to join our Intensive Community Wrap-Around Services Team! Under the clinical supervision of the Program Director and other clinically licensed team members, the Community Coach participates in the engagement, assessment and treatment planning process and as part of the team, provides interventions to address needs and goals of persons served. This paraprofessional position provides direct services to people with Autism Spectrum Disorder (ASD) and Serious Mental Illness (SMI) in community in the areas of housing, risk mitigation, transportation, skill-building, money management, medication administration and other areas based on the person centered treatment plan. Services are provided based on Recovery tenets and are aimed to promote overall health and well-being of persons served.
What you’ll do:
  • Engage people and partner with them to improve their health and wellness, achieve symptom stabilization and self-management, develop and/or restore skills impacted by mental illness and live a self-directed life.
  • Assist people with autism spectrum disorder, psychiatric and substance use disorders to identify personal priorities, preferences, strengths, and interests in order to help them establish goals that support a life in recovery.
  • Provide direct care services in timely and flexible ways.
  • Carries out the assigned interventions and action steps of the treatment plan, through the provision of direct support and rehabilitation services to people on an individual, group, and family basis in the office and in community settings to teach behavioral symptom-management techniques to alleviate and manage symptoms not reduced by medication and to promote personal growth and development by assisting people to adapt to and cope with internal and external stresses.
  • Collaborate with team in assisting each person in finding and maintaining a safe and affordable place to live - apartment hunting, finding a roommate, landlord negotiations, cleaning, furnishing and decorating, and procuring necessities (e.g., telephone. furnishings, linens).
  • Organize and lead individual and group social and recreational activities to structure each persons time, increase social experiences, and provide opportunities to practice social skills and receive feedback and support.
  • Other duties as assigned by the Program Director.
What you’ll need:
  • Education/Certification:
    • High School Diploma/GED equivalent required. Bachelors Degree highly preferred.
  • Experience:
    • In lieu of college degree, at least one year experience with population that has severe and persistent mental illness and/or substance use disorder.
  • Other requirements:
    • Must have valid Driver’s license. Must be MAP certified, CPR/first aide certified within five (5) continuous months of employment/date of hire.
