Job Summary and Qualifications

I. Position Summary: This position supports the mission of The Medical Center of Aurora/Centennial Medical Plaza of optimizing the quality of life of all those served by expanding beyond the traditional focus on medical needs to the needs of the whole person. The Social Worker II is responsible for assessing the psychological, social, cultural and/or financial needs that impact the high-risk patient’s health and recovery. Has the ability to utilize social services, counseling and clinical knowledge in the assessment of these needs. Responsible and accountable for coordination of services through an interdisciplinary process which provides a clinical and psychosocial approach through the continuum of care and balances individually identified patients and family needs with the efficacious and cost effective utilization of resources. Through concurrent case management, patients will be assessed to determine appropriateness of admission, continued hospitalization, as well as appropriate level of care. Discharge planning will begin at the time of (or prior to) admission, and reassessed ongoing throughout the course of hospitalization. Knowledge of payer source implications on discharge plan, resources and placement options to maximize patient’s successful disposition.

II. Position Requirements:

A. Licensure/Certification/Registration: Licensed Social Worker (LSW) preferred, not required.

B. Education: Master of Social Work (MSW) from accredited school of social work preferred. Master of Arts (MA) and / or (MS) in related mental field or Marriage and Family Therapy (MFT) from appropriate accredited school acceptable.

C. Experience: 3-5 years recent Case Management or Social Worker experience in an acute care setting preferred. Case Management experience in Mental Health setting preferred. Must be in process of obtaining Clinical licensure in specific discipline or open to supervision.

D. Special Qualifications: Must have excellent written, verbal and interpersonal communication skills and able to work with highly diverse population. Must be knowledgeable of State and Joint Commission standards. Must have ability to perform multiple tasks simultaneously and work in high stress environment. Must be able to apply critical thinking and formulate diagnostic impressions based upon review of systems, history, symptoms and current presentation. Must be able to provide crisis intervention and utilize system resources/referrals.

III. Degree of supervision required: Involves general guidance and direction by the Department Director or designee. Employee will be expected to perform all job duties independently and in accordance with established departmental and hospital policies and procedures.

IV. Populations and Ages of Patients Served: This position requires knowledge of growth and developmental needs of patients and families. Personnel in this position must recognize and respond to the behaviors of patients and families in each age category. The employee must be able to demonstrate the special skills required to care for patients and their families according to standards. (The skills and knowledge needed to provide such care may be gained through education, training or experience.)

V. Working Conditions: (Includes hospital rooms, clinical departments, common areas, non-clinical departments and hospital grounds). Potential hazards include:

VI. Physical Requirements: Requires corrected hearing and vision to within functional ranges. Requires frequent walking, standing, sitting, bending, and carrying tools and/or equipment up to 50 pounds and sufficient manual dexterity to operate therapeutic equipment. Must be able to stand for long periods of time. Must be able to handle behavioral issues of patients and visitors while under highly stressful and volatile situations.

VII. Personal Protective Equipment: This position requires the use of personal protective equipment during various task/activities. Personal Protective equipment includes, but is not limited to gloves, masks, safety glasses and gowns.

This role requires you to be fully vaccinated for COVID-19 based on local, state and /or federal law or regulations (unless a medical or religious exemption is approved).
