NURSE PRACTIONER - PEDIATRIC/ ADULT/ FAMILY PRACTICE
POSITION SUMMARY
The Nurse Practitioner (NP) provides well and acute/chronic care to patients in a primary care setting within the scope of their specialty. The NP is responsible for complete evaluation and documentation of the health status of his/her patients. The NP has a physician collaborator to support him/her as they provide this care. The NP participates in the Health Center on-call system where he/she will take the calls of patients/parents and when necessary refer to the pediatrician on-call or the emergency room. The NP is involved in the education of patients and families. The NP will practice to the full scope of his/her license to provide care to a diverse population.
QUALIFICATIONS
1) Current NYS licensure as a registered nurse.
2) Current NYS Certification as a Nurse Practitioner in their specialty.
3) Desirable (not required) current nurse practitioner certification by a national certifying body.

PHYSICIAN ASSISTANT
POSITION SUMMARY
The Physician Assistant (PA) provides acute/chronic care to adults in a Primary Care setting. The PA is responsible for complete evaluation and documentation of the health status of his/her patients. He/she has a supervising physician to support the care provided. The PA participates in the clinic on-call system. The PA is involved in the education of patients and families.
QUALIFICATIONS
1) Current NYS licensure.
2) Current NYS Certification a Physician Assistant.
Requirements
1. Assesses, diagnoses, monitors, coordinates, and manages health status of school-based patients over time; provides primary care, addressing social, psychological, and physical needs.
2. Maintains accurate patient electronic health records using appropriate format and legal terminology.
3. Provides anticipatory guidance to parent and child concerning child health and development and health promotion.
4. Carries out or modifies a predetermined immunization plan. Ability to provide injections.
5. Per protocol selects and recommends appropriate diagnostic and therapeutic interventions and regimes with attention to safety, cost, invasiveness, acceptability, and efficacy.
6. Performs, interprets, and monitors laboratory tests.
7. Assesses client/family need for intra-agency referrals (i.e., mental health, WIC, dental) and makes interagency referrals as necessary.
8. Participates in after hours call system.
