POSITION SUMMARY:
The Chronic Care Manager RN works to complete annual medicare wellness visits. He/she works as part of an integrated team with the Clinical Team, Population Health, Care Coordination, Providers, Clinic Administrators, Health Information Management Program Manager, and Billing to help improve the health, wellbeing, and satisfaction of patients (and their families).
QUALIFICATIONS:
Current New York State RN license with proven knowledge of patient care, access, and compliance regulations. A minimum of 3-5 years of clinical or quality program management experience preferred. Supervisory experience preferred. Has an excellent interpersonal and verbal communication skills. Is skilled with computers and computer applications. Gives attention to detail and possesses problem resolution skills. Has a positive attitude and high energy, is a quick learner, is adaptable to change and is resourceful and proactive when identifying and resolving problems. NYS driver’s license; reliable transportation; ability to lift equipment up to 30 pounds.
REQUIRED SKILLS:
Persuasion, excellent verbal communication, knowledge of health promotion and maintenance theories, building relationships and teamwork, conflict resolution, coordination, active listening, telephone triage, typing, knowledge of medical terminology, and program management.
