North Bay Regional Center is one of 21 Regional Centers for persons with developmental disabilities in California. Started as a grass roots movement, Regional Centers were originally established to assist persons with intellectual disabilities and their families in identifying and developing services and programs within their communities that help them lead more independent lives.

Each a non-profit, community-based organization, Regional Centers were established in 1965 under legislation sponsored by Assemblyman Frank Lanterman.

The Lanterman Act became effective in 1969 and later expanded the populations served to include persons with intellectual disabilities, cerebral palsy, epilepsy, autism, and other disabling conditions requiring services similar to individuals diagnosed with intellectual disabilities.

In 1972, NBRC was incorporated to serve Napa, Sonoma and Solano counties.

North Bay Regional Center is a nonprofit private corporation that contracts with the Department of Developmental Services to coordinate services and supports for individuals with developmental disabilities.

North Bay Regional Center serves over 10,000 people living within the geographic boundaries of Napa, Sonoma and Solano counties.

Position : Service Coordinator - Young Children Ages 3-6

DEFINITION :

  • Under the general direction of a Case Management Supervisor, responsible for the full range of case management duties including cases requiring intensive case management and high levels of expertise.
  • Implements the clients' service plans.
  • If assigned to support young children (ages 3-6), will be responsible for assisting Intake with the process of determining Lanterman eligibility for children under Provisional Eligibility criteria prior to turning 5 years old.

Nature of Work

  • Under the general supervision of a Case Management Supervisor , the Service Coordinator manages the delivery of services to regional center clients including those with intensive physical or emotional needs;
  • ensures that the client's Individual Program Plan and its objectives are pursued; assists clients in use of public and private resources before using regional center funds to purchase services;
  • monitors vendor services and verifies that services are delivered in accordance with the purchase of service authorizations;
  • evaluates the effectiveness of clients' Individual Program Plans; ensures that appropriate community resources are made available to the client;

conducts annual reassessments of clients' developmental status in conjunction with the interdisciplinary team; may conduct client assessments on an occasional basis .

Depending on caseload assignments, may be involved in the Lanterman eligibility determination for children under Provisional Eligibility .

Shall act occasionally, as assigned, in a supervisory or lead capacity, e.g., Officer of the Day or Acting Supervisor.

Minimum Education and Experience Requirements

A Master's degree in a field related to developmental disabilities.

Bachelor's degree in a field which offers content related to developmental disabilities, such as social work, psychology, counseling, a special education or nursing and three years employment in a field related to developmental disabilities.

Minimum qualifications may be waived depending on the agency’s need for specialized skills (such as language abilities).

Special Requirements

  • Fluency in a language in addition to English is desirable and may be required.
  • Possession of a valid California driver's license, and minimum legal automobile liability insurance required; the ability to provide personal automobile transportation during working hours and after hours emergencies or access to another form of reliable transportation which meets these minimum legal requirements.

$54,852 - $73,536 with opportunities for 5% increases annually
