Clinical Women’s Health- Behavioral Health Care Specialist -SW - Weill Cornell
The Clinical Pediatric Behavioral Health Care Specialist will work with a clinical team that advances behavioral health integration in the women’s health primary care setting using principles of the Collaborative Care Model.
Job Responsibilities
·Provides evidence-based behavioral health interventions and psychoeducation.
·Assesses and systematically collects and tracks data in a patient registry related to patient behavioral health status and works with the care team to develop the plan of care.
·Participates in-person or virtual huddles when shared patients are discussed.
·Utilizes data, including registries, to continuously meet care coordination needs across settings in conjunction with the patient, family and/or other care providers.
·Develops and maintains a working knowledge of community resources and managed care review processes.
·Assumes responsibility and accountability for effectively managing behavioral health care of individual patients.
·Documents history, assessments, and interventions to reflect comprehensive and integrated approach to patient care. Ensures complete, accurate and timely documentation in the electronic medical record.
·Identifies learning needs of the patient and families, provides patient education; educates by adapting to patient and family learning style utilizing the teach-back method.
·Attends team meetings and case conferences to present and discuss patient disposition and outcomes data.
·Assists with the coordination of care across the continuum. Assists with management of urgent and emergent patient care situations.
·Assists with the management of patients with multiple chronic health conditions, acute episodes of care, behavioral health challenges while promoting and supporting prevention and wellness.
·Provides care that is safe, ethical, patient- and family-centered, culturally congruent, and evidence-based.
·Performs other related duties as assigned and as appropriate to scope, education, and/or training.
The role is onsite, Monday - Friday, 9:00 AM – 5:00 PM

Licensure in Clinical Social Work (LCSW)
