Job Description:

The Clinical Counselor - LISW will work in collaboration with primary care providers, psychiatrists, and others to help patients with their mental health problems as an integral part of their overall health care. This may include psychosocial assessment, time-limited counseling appropriate to the primary care setting, consultation with and education of primary care providers about mental health issues, brief problem-solving and crisis interventions with patients in the primary care clinic, monitoring of patient progress, and at times referrals to outside mental health providers and other resources needed by the particular patient.

DUTIES PERFORMED

  • Function as a core member of an integrated care team that involves the patient, the patient’s primary care provider, a consulting psychiatrist, and other staff as appropriate.
  • Provide direct clinical care of patients in primary care, most often in scheduled psychotherapy visits, sometimes in brief interventions in the primary care clinic. Group therapy may be employed at times.
  • Conduct psychosocial assessments of referred patients with an emphasis on arriving at a working differential diagnosis of psychiatric and substance abuse problems and good understanding of the patient’s medical conditions, stressors, and social milieu.
  • Use time-limited, evidence?based techniques such as motivational interviewing, behavioral activation, problem?solving treatment, or other treatments appropriate for primary care settings.
  • Provide basic casework services as needed to link patients to needed resources and services.
  • Monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications.
  • Support psychotropic medication management prescribed by PCPs and psychiatrists, focusing on treatment adherence, side effects and other complications, and effectiveness of treatment.
  • Track patient progress, changing treatment or re-conceptualizing the patient’s problems for those not improving and reaching out to those not fully engaged in treatment.
  • Engage in regular consultation with the team psychiatrist with a focus on new patients and patients not making progress.
  • Complete relapse prevention plan with patients who are in remission.
  • Engage in brief interventions in the primary clinic for patients with acute problems.
  • Consult with and educate primary care providers about mental health issues.
  • Facilitate referrals to outside providers as needed for longer term or more specialized care.
  • Provide coverage to other New Horizon offices as needed.
  • Complete timely and appropriate billing for services.
  • Document work in shared electronic record that facilitates team communication and care plan.
