Job Description:

The Substance Abuse Counselor - LISW, Certified works under the general supervision of the Director of Behavioral Health. The Substance Abuse Counselor will work in collaboration with primary care providers, psychiatrists, and others to help patients with their mental health problems as an integral part of their overall health care. The Substance Abuse Counselor will provide assessment, treatment planning, individual, family, and group counseling for patients as it pertains to substance use disorders and opioid awareness. This position specializes in work with people experiencing mental health and substance abuse problems.

DUTIES PERFORMED

  • Responsible for responding to phone calls and inquiries from clients, physicians, or other referral sources requesting counseling services.
  • Provide accurate documentation in the Electronic Health Record.
  • Provide appropriate therapy and guidance related to a client’s specific symptoms.
  • Develop appropriate individualized treatment plans with specific goals and objectives, as well as measurable outcomes.
  • Teach individuals how to modify their behavior with the intention of full recovery.
  • Make necessary referrals to outside agencies.

Other duties

  • Participate in CQI, other internal committees, special projects/observances or activities that promote improvements in organizational performance and/or advance the mission, goals, and objectives of New Horizon Family Health Services.
  • Keep records of patient contacts and counseling activities in accordance with professional and program standards with attention to the need for data in relation to quality management, program planning and reporting. Prepares monthly progress reports and submits to the supervisor by the 5th working day of the following month.
  • Must hold all patient Protected Health Information (PHI) other patient personal information and agency information in confidence, in accordance with the Employee Confidentiality Statement.
  • Actively participate in and comply with all aspects of the NHFHS Corporate compliance Program, follow the Program Code of Conduct and obey all relevant laws, statutes, regulations and requirements applicable to Medicaid, Medicare and other State and Federal health care programs.
  • Obtain continuing education to remain current in professional discipline.
