NYTPS is hiring a Bilingual (Spanish) Early Intervention Ongoing Service Coordinator (OSC) for the Nassau and Suffolk County Early Intervention programs. This is an excellent opportunity for experienced professionals as well as new graduates.

Working as an OSC for NYTPS offers:

We are looking for energetic, motivated, and organized candidates who are committed to helping families in the community.

OSC/case management responsibilities include:

· Guide families through and help them understand the Early Intervention process.

· Coordinate Early Intervention evaluations and services for the child based on family’s and providers’ availability.

· Participate in the development, implementation, and monitoring of the Individualized Family Service Plan (IFSP).

· Maintain regular contact with families, service providers, and NYTPS staff.

· Monitor services and maintain quarterly progress
reports and semi-annual IFSP reviews or amendments.

· Maintain proper documentation of all contact made with families and Early Intervention
providers as well as any information pertinent to a child’s case.

Qualified candidates must meet at least one of the following education and experience requirements:

· Two years' experience providing service coordination

· One year of service coordination and one year experience in a service setting with
infants and toddlers with developmental delays or disabilities

· One year of service coordination and an associate degree in health or human services

· Bachelor’s degree in health or human services
