New York City Health + Hospitals/Kings County Hospital Center is Hiring a Social Worker Level V Near Brooklyn, NY

Tour II: 9:00 A.M – 5:00 P.M

PURPOSE OF POSITION:

The Social Worker Level V is an LCSW who functions as the Team Leader for the Kings OnTrack Program in the Adult Outpatient Department. The OnTrack NY Program Treatment Team is a multidisciplinary unit that works collaboratively with the individuals diagnosed with first episode psychosis (FEP) to achieve recovery goals. The Social Worker Level V oversees all aspects of the team functioning and provide administrative and clinical oversight of the program.

AREAS OF RESPONSIBILITIES:

Licensed clinician with minimum of 3 year of experience working clinically with individuals with mental health conditions and substance addiction. The Team Leader is responsible for overseeing the administrative operations of the OnTrack program and also functions as a Primary Clinician for a subgroup of clients.

SUMMARY OF DUTIES AND RESPONSIBILITIES:

The Social Worker Level V who functions as Team Leader for the grant funded Ontrack program will work full time as part of a small multidisciplinary team providing administrative oversight of the program and clinical tasks for patients in the earliest stages of illness. The clinician will be the direct supervisor for employees and be responsible for conducting performance evaluations and managing time and attendance of direct reports. The Team Leader will serve as the Recovery Coach and functions as a primary therapist for a small caseload. The candidate should have a knowledge base of early psychosis symptoms and various treatment modalities. The duties include but are not limited to the following:

  • Monitor caseload size and coordinate program intakes and discharges. As the communication hub of the team, ensure all team members are kept informed of relevant issues involving participants and their families.
  • Serve as Primary Clinician for a small subgroup of participants and their families.
  • Function as coordinator and mediator for the OnTrack team, participants and their families.
  • Management tasks, including reviewing patients charts to assure compliance with OMH standards, registration and tracking, program census and visit tracking reports.
  • Address and support staff around crisis situations, reporting and de-escalation
  • Recruiting patients for program through community engagement, education and presentations at potential referral sources
  • Establishing and maintaining database of referral sources with up-to-date contact information
  • Oversee the process of conducting telephone and in-person eligibility screening of patients referred to program⁠⁠⁠⁠⁠⁠⁠.
  • Offers to meet weekly with individual consumers and families, focusing on the individual psychosocial needs of each patient and family, motivating progress, instilling hope and promoting successful living and help in attaining developmentally appropriate milestones in the context of severe mental illness⁠⁠⁠⁠⁠⁠⁠.
  • Serves as member of the team, implement behavioral interventions and takes an active role in the family support domain⁠⁠⁠⁠⁠⁠⁠.
  • Conducts clinical assessment to determine the participant's level of social functioning and substance use via a shared decision making (SOM) framework.
  • May provide clinical support to participants and their family through community visits⁠⁠⁠⁠⁠⁠⁠.
  • Participates in and provide ongoing training and supervision to direct reports.
  • Attends weekly team meetings.
  • Rotate on-call responsibilities with other team members.

Job Type: Full-time

Pay: $101,439.00 per year
