You’ll work directly with patients or speak with them on the phone or online to help meet their needs and contribute to a positive care experience. Culturally responsive care incorporates an awareness of potential barriers to accessing services and care that may result in poor health outcomes.Two (2) years of experience addressing racial health disparities and improving health outcomes of Black patients. Implementing care delivery and social support models aimed at achieving health equity for the Black community.Two (2) years of experience implementing or providing direct services/support through or in partnership with organizations that serve the Black community.EducationGraduate of accredited school of nursing.License, Certification, RegistrationThis job requires credentials from multiple states. Credentials from the primary work state are required before hire.
