Position: Children's Care Manager

Position Summary: The Children's Care Manager role is an opportunity to provide Health Home Care Management Services for youth aged 0-21. This role takes a holistic approach to coordinating services based on individual needs, utilizing a strength-based, trauma-informed, and family-driven approach. The Care Manager engages with the youth and family to ensure they play an active role in the care plan's development and execution, linking them with resources and services, promoting evidence-based wellness and prevention, and providing comprehensive transitional care. Minimum Education and/or Professional Qualifications/Skills:

Bachelor’s degree in a human services and two years of experience working with youth in a community setting; or
Master’s Degree and one year of experience in one of the above listed fields.
