New Avenues to Independence is Hiring a CLE (Customized Living Environment) Manager-New Tech Near Brecksville, OH

As a member of the leadership team, the Customized Living Environment (CLE) Home Manager is responsible for maintaining active treatment delivered in a group home, providing support, guidance, and supervision to CLE Support Specialist staff in the provision of basic care and implementation of person-centered services and goals. Conducts training and coaching as necessary for the CLE Support Specialist assigned to the home.
Essential Duties and Responsibilities include the following; other duties may be assigned:
Person Centered Planning and Reporting:
1. Ensure Health and Welfare of individuals served through direct services provided.
2. Maintain knowledge of I.P. and other service delivery techniques
3. Schedule community errands/trips appropriate to individual’s interests and needs.
4. Monitor proper diets, meal planning and food shopping.
5. Maintain and secure all financial information and monies for the individuals as required by their Person- Centered Plan (PCP) /I.P. and policy/procedure.
6. Assist with developing a comprehensive PCP/I.P. through a team process to meet the individual needs of those served.
7. Monitor, implement, and document each individual’s active treatment programming and various interventions per I.P. goals and objectives.
8. Coordinates, monitors, and ensures all aspects of individual’s I.P. are implemented and properly documented.
9. Report and investigate all Unusual Incidents.
10. Assist in facilitation of MUI investigations.
Maintain customized living environment:
1. Ensure environment customization optimizes independence.
2. Ensure training of all integrative adaptive equipment, both mechanical and technological.
3. Monitor customized devices and report any repairs needed.
Supervision:
1. Ensure all Support Specialists are trained in PCPs/IPs.
2. Reports any conduct performance issues/actions to direct supervisor on a timely basis.
3. Follow New Avenues ICF on call procedures.
a. Maintain appropriate staffing
b. Available to work direct shifts as needed
c. Maintain timely communication with all team members.
4. Maintain effective relationships with team members to ensure the following:
  • Orient new staff to the household routines and document on-site and individual specific training of new employees.
  • Ensure smooth daily transition between working varying shifts in order to facilitate supports for individuals.
  • Schedule community errands/trips appropriate to individual’s interests and needs.
  • Ensure all incident reporting procedures are followed including incident reports being reported and written immediately after incident occurs; then sent in a timely manner.
  • Maintain and secure all financial information and monies for the individuals as required by policy/procedure.
  • Communicate with internal and external support members (guardians, surveyors, county and state officials).
5. Ensure continued staff development and participate in the evaluation and recognition of support staff and team.
Administrative Duties:
1. Timely completion of all documentation deadlines and staff accountability. Including but not limited to:
  • Progress notes/Goals and Service Support documentation/Site documentation
  • 30 Day Site Training, Annual training and all required trainings.
  • Monitoring of documentation and implements change over
  • Assist with coordination of medical appointments and ensures written documentation/physicians orders are relayed timely to appropriate staff.
  • Communicate all recommendations from medical appointments to the core team (including IS) and staff.
  • Create and publish monthly schedule in Data plus per the staffing ratios and budgeted hours.
2. Review weekly payroll for staff hours and make adjustments as necessary.
3. Participate in survey and inspection preparation with attention to compliance with expectations on a daily, ongoing basis.
4. Secure the house and maintaining a healthy and safe environment by:
  • Ensuring the house cleanliness on a daily basis.
  • Monitoring proper diets, meal planning and food shopping.
  • Communicating appropriate repairs and equipment needed by submitting a work order to facilities department.
Evaluation Factors/Qualifications/Physical Requirements:
Evaluation Factors:
1. Ensure the staff schedule in Data Plus meets the staffing pattern of the site and I.P.
2. Ensure that all staff time and attendance entries are complete in Data Plus by payroll deadline.
3. Ensures all documentation listed in responsibilities is completed and submitted in a timely manner.
4. Attend all mandatory staff trainings to enhance personal competencies of the individuals and agency.
5. Ensure completion of all site specific/individual specific trainings are complete for new employees at site.
6. Monitor and document progress with PCP/I.P. goals and client-specific technology to ensure increased independence and individual goals are met in a timely manner.
Qualifications:
1. Must be 18 yrs. of age or older.
2. Associate Degree in a field related to human services and four years full-time work experience providing direct care services medically fragile individuals. LPN preferred.
3. Possess management/supervisor skills.
4. Must have reliable transportation available at all times.
5. An ability to stay organized while managing multiple tasks.
6. A valid Ohio driver’s license with 4 points or less on the individuals BMV driving record
7. Maintain auto liability insurance with at least state minimum coverage level.
