ROLE OVERVIEW and PURPOSE

The Registered Dietician (RD) will design and implement nutrition education programs at Neighborhood Healthcare. This role will evaluate participant nutritional status, develop/implement nutritional care plans, and communicate nutritional information to providers and other health professionals. Additionally, this role will provide dietary counseling and education to patients.

RESPONSIBILITIES

· Conducts individual nutrition assessments to establish nutrition care plans using medical data to determine health risks and needs for patients, including therapeutic diets

· Provides diabetes education to patients, including risk factors, signs/symptoms, medical nutrition therapy, blood glucose monitoring, and medication management

· Evaluates patient glucose logs and assess medication dosing adjustments, as needed

· Supports individual, family, and group nutrition/chronic disease education services by creating culturally appropriate curriculums, planning programs, delivering education, and evaluating effectiveness

· Serve as a resource for health education staff regarding American Dietetic Association Standards and CPSP or PACE guidelines

· Participates and contributes to health education projects, staff trainings, and meetings

· Keeps department updated with American Dietetic Association and American Diabetes Association standards as it relates to nutrition and disease management

· Conducts chart reviews to ensure compliance with CPSP or PACE program guidelines, as needed

· Plans, creates, and develops nutrition and health education topic outlines and curriculums by incorporating various learning techniques and formats, such as group discussions, lectures, role-playing

· Reviews and provides action-oriented feedback regarding the efficacy of materials and programs

· Maximizes utilization of schedules by proactively managing patient visits, including scheduling follow-up appointments, calling no-shows, pulling recall reports

· Impacts patient experiences by demonstrating courteous and helpful behavior and a commitment to accuracy

· Shares accountability for overall patient health outcomes with care teams

· Documents all relevant aspects of nutritional care for patients in their medical records in accordance with Neighborhood standards, including documenting participant changes and communicating with providers

· Maintains and follows established departmental policies and procedures, objectives, quality assurance program, safety, environmental, and infection control standards

· Enhances professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops

· Participates in required meetings and committees, as directed

EDUCATION/EXPERIENCE

· Bachelor’s degree in nutrition or dietetics required; master’s degree in nutrition preferred

· Valid registered dietitian license required from the Academy of Nutrition and Dietetics

· National certification by commission on dietetic registration required

· Certified diabetes educator certificate preferred

· Valid BLS certification from approved American Health Association provider required upon hire

· One year registered dietitian experience preferred

· Experience in nutrition and/or chronic education or related field preferred

· Bilingual (English/Spanish) preferred

For Prenatal Only:

· Lactation educator or training specialist preferred

ADDITIONAL QUALIFICATIONS (Knowledge, Skills and Abilities)

· Excellent verbal and written communication skills, including superior composition, typing and proofreading skills

· Ability to interpret a variety of instructions in written, oral, diagram, or schedule form

· Knowledgeable about and experience with electronic medical records systems

· Knowledgeable about and experience with current nutritional science theories and best practices

· Knowledgeable about and experience with adult and child education techniques, practices, and tools

· Knowledgeable about community resources and health/social service systems in San Diego and Riverside Counties

· Ability to translate best practices into meaningful and culturally appropriate resources

· Ability to work effectively with patients from diverse social, cultural, and economic groups

· Ability to successfully manage multiple tasks simultaneously

· Ability to communicate proper nutrition driving health to patients

· Excellent planning and organizational ability

· Ability to work as part of a team as well as independently

· Ability to work with highly confidential information in a professional and ethical manner

Physical Requirements

· Ability to lift/carry 50 lbs/weight

· Ability to stand for long periods of time

Pay range: $40.73 - $49.79 per hour, depending on experience.
