Position Summary:

The Registered Dietitian is responsible for completing individual nutrition assessments and providing nutrition counseling and group health education at both clinic sites. This full-time position will work closely with NARA’s Chronic Disease Program, which includes the Breast & Cervical Cancer Early Detection Program, WISEWOMAN Program, Colorectal Cancer Screening Program and the Diabetes Treatment and Prevention Program.

This position will serve as a subject matter expert in diabetes treatment and prevention and will lead/coordinate the Diabetes Prevention Program and Diabetes Self-Management Education group health education classes. This will include outreach, scheduling, counseling, tracking and reporting outcomes.

This position will also create, conduct, and implement health promotion activities such as health fairs, walking programs, cooking presentations, and employee health activities.

What you will do:

  • Work collaboratively with other care providers to improve the health of NARA’s patients by: assessing nutritional needs, developing individualized nutrition care plans, providing counseling, health education and support to patients, families, and caregivers, and monitoring/re-evaluating the patients’ tolerance and response to the nutrition care plan.
  • Serve as a subject matter expert in diabetes treatment, prevention and screening.
  • Accurately document nutrition assessment and services provided in Electronic Health Records and Chronic Disease Case Management database.
  • Take and evaluate patient vitals including blood pressure, capillary blood glucose, weight, height, and point of care lab measures including A1C and Cholesterol
  • Recruit for and lead Diabetes Self-Management Education and Diabetes Prevention Program group classes
  • Provide guidance to patients when goal setting and creating sustainable behavior change plans.
  • Coordinate with other team members to address social determinants of health and navigate patients to follow-up care when referred services are necessary.
  • Lead healthy cooking demos and walking program
  • Track, report, and manage diabetes data.
  • Assist with preparing reports for funding agencies.
  • Use evidence-based communication strategies (ie. Motivational Interviewing)
  • Communicate with patients by phone, email and text to provide support and encouragement
  • Develop culturally sensitive recommendations which take into consideration socioeconomic status and personal preferences
  • Manage provider Chronic Disease Program referrals in the electronic health record in a timely manner. Schedule appointments as needed.
  • Contribute to open team forums by listening, sharing ideas, and creating plans for program improvement
  • Coordinate program events: request approval for supplies, purchase, pick-up/set-up and take down as needed.
  • Provide outreach at community events such as the NARA NW New Year’s Eve Powwow, Delta Park Powwow, health fairs and Saturday farmers markets etc.
  • Registered Dietitian with license to work in Oregon and Washington, or eligible to apply for Oregon/Washington dietitian license and 2 years’ experience working in a clinical setting, preferably with people with diabetes.
  • Flexible schedule – position requires some travel, evening, and weekend work.
  • Proficient computer skills including Microsoft Office Excel, Word and PowerPoint
  • Must have access to a reliable vehicle and a valid drivers’ license and be willing to travel between 2-3 different sites within the Portland Metro area
  • Must have ability to engage and persuade clients and have experience providing nutrition education and counseling to patients, families, caregivers, and groups from diverse socioeconomic and cultural backgrounds.
  • Ability to walk, sit and stand for prolonged periods of time.
  • Ability to use hands to finger, handle, or feel; and reach forward with hands and arms.
  • Ability to twist, bend stoop, kneel, or crouch.
  • Ability to lift and/or move up to 25 pounds
  • Minimum of two years sobriety/clean time for NARA employees in recovery.
  • Pass a background investigation for all potential hires to determine past history of sexual and child abuse
  • Pre-employment drug screen for all potential hires.
  • Experience working with the Native American /Alaska Native community is preferred.
  • Experience working as a Lifestyle Coach with the National Diabetes Prevention Program is preferred.
