Native American Rehabilitation Association is Hiring a Case Manager - Assertive Community Treatment (CADC) Near Portland, OR

The CADC Case Manager will provide case management and addictions counseling services, including individual and group counseling. Responsible for completing ASAM assessments, and for coordinating referrals to residential or intensive outpatient chemical dependency treatment services. Also assists clients in different stages of change in relation to substance abuse, focusing on building motivation.

RESPONSIBILITIES:

  • Work with clients in their homes and in the community as well as in the office.
  • Ensure timely documentation and reporting as required.
  • Seek and accept clinical and administrative supervision regularly and integrate feedback into daily performance.
  • Engage in collaborative relationships with ACT team members and staff across NARA programs to work towards integrated systems services.
  • Provide screening, evaluation, and referrals for all clients as per assigned.
  • Formal and informal ongoing clinical and non-clinical assessments of clients within the appropriate scope of practice; developing, writing, implementing, and revising treatment plans with clients
  • Assist clients in obtaining income and housing
  • Must run one EIMR group per week at minimum and organize material, books, slides, films, and equipment needed for group sessions scheduled.
  • Assist client in scheduling and attending medical and psychiatric appointments as needed
  • Build relationships, serve as a liaison between the client and other community services
  • Provide crisis intervention as needed
  • Participate in on-call service coverage.
  • Reliable transportation is required.
  • Attend required meetings including weekly staff meetings and daily morning ACT meetings.
  • Attend all necessary/required trainings.
  • Perform other duties as assigned.
  • Bachelor’s Degree in social work, psychology, or other social services related field
  • QMHA required (QMHP preferred)
  • CADC required
  • At least one year experience providing direct services to individuals dually diagnosed with mental health concerns and substance use issues
  • Experience working within the American Indian/Alaska Native population
  • Minimum of two years sobriety / clean time, if in recovery.
  • Ability to pass a background investigation for all potential hires in order to determine any history of sexual and child about and / or any other relevant history.
  • Ability to pass a pre-employment drug screen for all potential hires.
