Position Summary

The Remote Sub-Lead Case Manager is responsible for coordinating case management and safe and timely release service, training new case managers, and supervising the work of other case managers. Remote Sub-Lead Case Manager will maintain a reduced case load and assist the Remote Lead Case Manager with responsibilities to provide 7-day case management, which requires working a flexible schedule including early or late hours and weekends, and be on-call.

RESPONSIBILITIES

· Perform all work in a manner consistent with the National Youth Advocate Program’s mission, values and philosophies.

· Remote Sub-Lead case manager will be assigned to a team of remote case managers to ensure continuity of care between 8 a.m. – 10 p.m., 7 days per week. Remote sub-lead case managers will have full access to cases to ensure documentation, assessments, and sponsor communication continues and is completed in accordance with policies and deadlines for timely unification. The assigned case load will be reduced to allow for sub-lead duties.

· The remote sub-lead case manager provides site support to the on-site case management teams by providing all case management duties including such things as intakes, assessments, uploads to portals and electronic health record systems, coverage, and other case management duties based on the needs of the local sites.

· Document all actions taken and contacts with youth, sponsor, and stakeholders in the form of progress notes and ensure notes are uploaded timely to portals and electronic health record systems.

· Provide supervision, mentoring, case consultation, and support to the remote case management team daily.

· Complete all required training and documentation in a timely manner and assist the Remote Lead Case manager to ensure the remote case management team is trained.

· Establish and maintain a strong relationship with on-site cross-functional teams.

· Assist the Remote Lead Case Manager to guide and support the remote case management team with intakes, assessments, uploads to portals and electronic health record systems, coverage, and other duties as assigned based on the needs of the local sites.

· Assist the Remote Lead Case Manager by participating in Continuous Quality Improvement and case audit activities regularly.

· Assist the Remote Lead Case Manager by actively participating in the regular supervision process.

· Provide nationwide remote site support and supervision of intakes, assessments, uploads, coverage, and other duties as assigned based on the local sites’ needs.

· Ensure all policies are adhered to in a timely manner.

· Perform other duties as requested.

MINIMUM QUALIFICATIONS

· Master's degree in behavioral sciences, human services, or social services fields or Bachelor's degree AND at least 3 years progressive employment experience that demonstrates supervisory and case management experience.

· A valid professional license as required by state or contract regulations.

· Bilingual English/Spanish fluency required.

· Required to work flexible non-traditional hours to facilitate program services, including working on-call, overnight, weekends, and evening schedules.

· Cleared Level II background check from appropriate entity.

· Must be at least 21 years of age at the time of hire.

· Proficient use of desktop and laptop computers, internet search, people management programs, smart phones and tablets, printers, fax machines and photocopiers as well as software including word processing, spreadsheet, and database programs.

· Valid driver’s license, reliable transportation, automotive insurance, and a good driving record is preferred for advancement purposes.
