Responsibilities:
- Coordinate and oversee the completion of the Minimum Data Set (MDS) assessments for patients in a hospice care setting.
- Ensure compliance with regulatory standards and guidelines related to MDS assessments.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Conduct regular assessments of patients' physical, emotional, and psychosocial needs.
- Coordinate discharge planning and facilitate smooth transitions of care.
- Utilize knowledge of anatomy and medical terminology to accurately document patient information.
- Collaborate with ICU staff to ensure accurate and timely completion of MDS assessments.
- Utilize Epic or other electronic health record systems to maintain patient records.
- Collaborate with managed care organizations to ensure appropriate reimbursement for services provided.
- Provide case management services to ensure continuity of care for patients.

Requirements:
- Bachelor's degree in nursing or a related field preferred.
- Valid LPN license required.
- Minimum of 2 years of experience in case management or patient care coordination, preferably in an acute care setting.
- Strong knowledge of MDS assessment process and regulatory requirements.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively with interdisciplinary teams.
- Strong organizational and time management skills.
- Proficiency in using electronic health record systems, such as Epic, preferred.

Pay: $38.00 - $40.00 per hour
