Position SummaryThe Housing Specialist (HS) is responsible for identifying, securing and maintaining permanent housing for HHCC clients who are homeless or at-risk of homelessness. The HS is responsible for gathering relevant documents, completing housing applications to assist clients, and coordinating with realtors, landlords, housing providers, case managers, and supervisor/s.
Essential Functions• Collaborates with primary Care Manager around client's housing needs and goals as well as exploring best approaches to securing stable housing. Collects data and/or documentation from case management teams, such as referrals, clinical information, financial records, and others as needed to draft and submit comprehensive application packets and/or data reports, such as collecting monthly placements achievements per team, etc.• Completes housing applications for permanent housing or housing subsidies, including any voucher/rental programs. Collects and submits related documentation in a timely and accurate manner.• Escort clients to community based organizations to assist with housing needs and follow up (i.e. housing court, Homebase, rental assistance agencies, HRA, storage facilities). Performs site visits to locations including clients' transitional homes, prospective permanent housing locations, treatment programs, or employment site. • Develops, follows, and documents daily schedule of appointments. Informs supervisor or other management staff of changes to schedule or appointments. Maintains accurate timesheets.• Collaborates with team members of other professional disciplines, including vocational counselors, social workers, treatment providers, and housing specialists, to ensure that clients are meeting goals and objectives of the Comprehensive Housing Plan.• Documents manually and electronically housing assessment, referrals and applications, client's status, progress with goals and referrals, contacts with client and service providers, services and interventions provided, and meetings. Adheres to documentation procedures.• Performs other duties as assigned.
Knowledge, Education & ExperienceFormal Education High SchoolExperience 1 YearCriteria that would be desirable but not requiredMicrosoft Office is required, bilingual English/Spanish is a plus, and knowledge of electronic recordkeeping systems, such as Word, Outlook, PACT Housing System, CAPS, NYC Housing Connect, NYCHA, etc. Experience working with clients living with substance use, behavioral health, and/or unemployment challenges.Minimum Knowledge Requires thorough knowledge of a specialized or technical field. May compile objective and measurable data and produce reports. May require operating knowledge of manufacturing or computer equipment, OR may require broad knowledge in clerical or service areas.Updating of Knowledge The knowledge required to perform this job once learned has frequent changes, but the changes are relatively minor.Additional Comments on Knowledge, Education, and ExperienceKnowledge of housing resources. Ability to research, network, and identify housing options. Ability to work with realtors/landlords/housing providers/vendors. Knowledge of client skills needed to obtain housing.
