Overview

Responsibilities

Position SummaryThe MWRAP Bilingual Care Manager (BCM) works within the community of NYC to locate, educate, assess and enroll pregnant women and new mothers, who have a Substance Use Disorder, and are in need of Case Management services to facilitate their recovery, and the healthiest possible delivery or care of their child. The MWRAP BCM provides assessment, treatment planning, and service coordination activities for a caseload of eligible clients, with a maximum capacity of 35 clients. The MWRAP BCM is also supported by one Peer Recovery Specialist, who assist in the execution of the treatment plan. The MWRAP BCM works closely with other health and social service providers, including the Administration on Children's Services (ACS) to ensure comprehensive and appropriate care needs are met to stabilize participants, promoting access to health and wellness while reducing the risks of substance use to mother and child.

Essential Functions• Monitor each participant's progress in their recovery, and adherence to the agreed upon treatment plan through a variety of in-person community based encounters, the supervision of peer supports, telephonic guidance and support, and appropriate coordination with other providers. • Complete client-centered comprehensive functional assessments to identify the medical, behavioral health, and social needs/goals of each client, keeping at the forefront the client's desire to deliver and raise a healthy child, and be a healthy parent.• Develop, review, and update person centered treatment plans that are driven by client goals, an ultimate goal of recovery, and the desire to birth and/or parent a healthy child.• In addition to clinical treatment planning, and work toward recovery, assist clients in meeting their social determinants of health needs, such as securing entitlements and benefits, exploring avenues for increased income, seeking out healthy social supports, and seeking out resources to address housing instability.• In concert with the supervisor of the MWRAP project, and the Peer Recovery Support Specialist, present to area providers, and educate potential clients on MWRAP services; assess interested women/child bearing parents for eligibility, and if appropriate enroll into programming.• Maintain collaborative relationships with referring service providers, and develop new and meaningful partner relationships with all providers involved in the care of the client caseload.• Participate in individual supervision by preparing an agenda in advance, being prepared to discuss personal professional areas for development, specific client needs and assessments, and seeking out supportive supervision for critical or unmet needs. • Utilize NADAP database tools to maintain documentation and all relevant treatment records, entering contact notes within the time frame outlined in the Program Manual guidelines.• Performs other duties as assigned.

Qualifications

Knowledge, Education & ExperienceFormal Education Bachelor's DegreeExperience 3 YearsCriteria that would be desirable but not requiredMaster's Degree in Social Work or a related field preferred

Additional InformationThe MWRAP BCM is part of a team working on a grant funded program. The roll-out and implementation of this program may have moments of change, transition, program modeling. The best candidate for this role will embrace this role and take advantage of the rare opportunity to help build and sculpt a program from the beginning stages.The right candidate will be empathetic and caring towards the client population served, must have a client centered work strategy, and be excited to work with a pre-recovery and new recovery MAT (Medication Assisted Treatment) enrolled client.

Salary: $52,000 annually
