Job Description Summary

The MUSC Health Rena N. Grant Sickle Cell Center is seeking a nurse coordinator to help work with our dynamic team into the future. We are looking for an individual who is motivated to build relationships with both team members and patients to enhance clinic efficiency, education, community outreach, and research to improve our patients' outcomes.
The Sickle Cell Care nurse coordinator will participate as an expert practitioner in a variety of direct and indirect patient care activities that will enhance the overall quality of clinical care of the general sickle cell population. This position will directly report to the Ambulatory Service Manager for Sickle Cell.

Job Description

The Sickle Cell Care nurse coordinator is a registered nurse who is accountable for effective coordination of the sickle cell patient with their outpatient and inpatient care. This position systematically triages patients based upon clinical problems and educational needs, provides coordination of care for the patient, and is also a resource to clinic staff in regard to the direct provision of care. This individual will be an active member of the multidisciplinary team and collaborates in the development, execution, and evaluation of the multidisciplinary plan of care. This includes the establishment of goals and strategies for meeting the continuing care needs of the patient, family, and/or other care provider. This position works with the team to assure that systems of care are in place for patients to move across the continuum of care. This includes development of patient care protocols, order sets, and systems that allow care to take place in an effective and efficient manner. This position also puts systems in place to evaluate the effectiveness of patient care.

  • Patient care: Responsible for having primary role in the coordination of care for general sickle cell patients. Assists in the development of a patient intake, follow up and triage workflow for sickle cell patients. Participates in the evaluation and treatment planning of patients in the outpatient clinic including new referrals, routine follow-up visits, hospital follow-up visits, and acutely ill patients. Coordinates outpatient consultations, therapies, and treatment plans. Serves as an advisor for nursing staff when contacted by patients for management of medical problems. Identify resources available to support patient needs, which includes providing information on community resources and facilitating patent access to basic needs. Serves as the voice of the patient across care settings and empower the patient to problem-solve options of care and promoting the patient’s right to self-determination.
  • Education: Develop education resources for patients in concert with the supervising physicians to empower patients and caregivers for improved quality of life and adherence to complex medical regimens. Develop materials to highlight our clinical research portfolio for both patients/caregivers and referring physicians. Serve as a resource to identify potential patients eligible for clinical trials and collaborate with MUSC research staff to offer opportunities to our patients. Serve as a resource to clinic nursing and CMA staff on common adverse events in our population and ensure standardized approach to the management of these issues in collaboration with the supervising physicians.
  •  Coordination of Clinic: Coordination of new patient referrals in collaboration with central and sickle cell center scheduling to ensure efficient access with the appropriate provider is a key responsibility. Review new patient visit planning and scheduling of testing to ensure correct information and appropriate providers are scheduled to see patients. Oversight of clinic standardization of clinic orders for new referrals and return patients in collaboration with current resources. Collaborates with inpatient and outpatient hematology teams to facilitate patient transitions before and after hospitalization. Communicates with infusion nursing staff, sickle cell hematology teams, primary care and ED teams, and others as needed to follow up on care plans. Follows up on care plans with a focus on symptom management after clinic visits with phone call or in person visits. Supports patients and families in overcoming barriers to care and facilitates successful clinic visits through appointment reminders, arranging transport and/or use of video visits as appropriate. Ensures proper procedures are followed for sickle cell disease and pain management. Multiple resources are in place including clinic scheduling, nursing staff, and PharmD experts that will benefit from continuity across multiple providers through this position. Medication management including prescription refills, monitoring for drug toxicity, and prior authorizations will comprise a portion of this time. Quality improvement projects, determined in collaboration with the supervising physician, to ensure the clinic provides standardized efficient care across this population will also be a priority. • • Communication and Outreach: Collaborate with MUSC marketing to enhance outreach through the state and region of our program.

Additional Job Description

Required Minimum Training:

Completion of an accredited ADN/BSN Program and Current Unencumbered SC or Compact State RN Licensure

Required Experience:

Minimum of 1 year RN experience required.

Preferred Training and Experience:

Previous Hematology/Oncology or Sickle Cell experience preferred.

Required Licensure, Certifications, Registrations

All certifications must be current and complete prior to start date:

Basic Life Support (BLS).

Current Unencumbered SC or Compact State RN Licensure
