Community Health Worker
Position Responsibilities
Assist patients with socio-economic issues impacting their overall health.
Patient education on chronic disease management and reduction of future chronic health risks.
Facilitate communication and coordinate services with community resources such as food clothing, life skills classes, food, parenting, and others.
Collaborating team member with providers, nursing staff and others, serving as a supportive extender beyond Clinic visits to ensure patient healthcare needs are met.
Use of electronic health record to document patient goals and services.
Program reporting as required.
Position does require community travel and patient home visits. Position will require driving the Mobile Clinic to local communities for services.
Knowledge of community agencies and resources.
Education/Experience
Bachelor's degree in a social service field. Previous working experience in a healthcare environment required.
Requires current TN Driver's License.
Job Type: Full-time
Pay: From $17.00 per hour
Expected hours: No less than 40 per week
