• SUMMARY
• This role is responsible for completing and managing both Medicaid and Level of Care recertifications for the Palm Beach PACE Program. PRIMARY FUNCTIONS
• Completes Medicaid recertifications monthly.
• Completes assigned LOC 701B Assessments monthly.
• Completes all paperwork and documentation needed for the 701B assessments and Medicaid recertifications.
• Faxes or emails completed assessments to the CARES Office in DOEA and sends completed Medicaid applications to DCF.
• Enters the completed data on the 701B & interim review spreadsheet folder located on the shared R Drive
• Maintains open communication with DCF and social security officer to ensure the completion of Medicaid recertifications.
• Upload all necessary documentation into Cornify -- Pace logic.
• Reports to the Regional Director of Enrollment, any delays or issues completing the 701B assessments promptly.
• Ability to complete a thorough assessment/evaluation of a participant and their home environment.
• Follows instructions from an equipment’s instructional manual.  Reads maps, street finders and to follow written and oral directions.
• Performs all other duties as assigned by manager. SUPERVISORY RESPONSIBILITIES None
• QUALIFICATIONS (EDUCATION and/or EXPERIENCE)
• 16hrs DOEA certification training, preferred
• Experience in Cognify – Pacelogic, preferred
• Have access to a car as this job requires a face to face visit in order to complete the assessment, so the staff member will need to go to the participant’s home as needed

Job Type: Full-time

Pay: $19.00 - $20.00 per hour
