SUMMARY Responsible for the development and implementation of homecare services for program participants. PRIMARY FUNCTIONS Assess the homecare needs of PACE participants, and develop specific plans of care.
Maintain an awareness of participant / family / caregiver needs and problems. Coordinate and authorize all durable medical equipment, consumable supplies, environmental adaptations and other home services.
Coordinate scheduling of outside agency staffing and agency education, specific to PACE requirements. Perform computer entry to document participant care and demographic data, timesheets, care plans and reports.
Actively participate in Interdisciplinary team meetings. Review invoices for homecare services. Maintain vendor information.
Conduct annual homecare service agency audit. Process service requests for homecare needs as required. Facilitate coordination, delivery and pickup of DME / supplies.
Ensure Vendors receive complete and accurate information to facilitate service delivery including items needed and ordered, date and initial order number, time needed and other special information.
Liaise with other community services and resources to ensure that other resources are identified and initiated, if appropriate.
Provide information and education to individuals and groups in the community, as required and requested. Assist with PACE promotion in the community and intake duties for new enrollees.
Perform other related duties as required Supervisory Responsibilities This position may include oversight of Certified Nursing Assistants (CNAs).
QUALIFICATIONS (EDUCATION and / or EXPERIENCE) Bachelor’s Degree preferred. High school diploma or general education degree (GED) required;
one to three years related experience and / or training; or equivalent combination of education and experience. At least one year experience working with the frail elderly is required.
