Job Overview:
We are seeking a compassionate and dedicated Registered Nurse to join our Home Care Team. The ideal candidate will provide high-quality medical care to patients in the comfort of their own homes.
Responsibilities:
- Conduct patient assessments and develop individualized care plans
- Administer medications and treatments as prescribed
- Monitor patient vital signs and condition, and report any changes to the healthcare team
- Educate patients and their families on managing illnesses and injuries
- Collaborate with physicians and other healthcare professionals to ensure coordinated patient care
- Maintain accurate medical documentation
Requirements:
- Valid RN license in the state of practice
- Previous experience in areas such as home care, memory care, triage, acute care, or primary care is preferred
- Ability to Provide Training if needed
- Strong medical documentation skills
