Job Description
Job Title: MDS
Reports To: Administrator
JOB SUMMARY:
The purpose of this position is to foster the relationship between patients and the facility. This position participates in the admission process by assisting in the establishment of the resident’s eligibility for care, communicating with families and interacting with a wide range of staff members and physicians on behalf of the facility. Works closely with the marketer, the admissions team and social worker.
RESPONSIBILITIES AND MAJOR DUTIES:
1. Assist in the facilitation of the referral from an acceptable referral source ensuring continuity of the transitions of care/service.
2. Assist in the collection of resident related information for admission into the facility as a resident.
3. Follow up with potential referral sources and document appropriately.
4. Work with or in place of Marketing Director to cultivate positive community relations with referral sources in and around the community.
5. Educates and explains to patient/caregivers regarding facility services and assist in obtaining the proper paperwork as directed.
6. Attend community and agency meetings, functions and other programs to learn and to educate others regarding the services and support provided by the facility.
7. Assist in following through for orders or other documentation requiring physician attention/signature.
8. Attends all required meetings within and involving the facility.
9. Observe infection control procedures.
10. Ensure the Resident Assessment Instrument (RAI) process is followed in accordance to State & Federal Guidelines.
11. Follow Residents’ Rights policies at all times.
12. Adhere to all facility policies & procedures and ensure they are adhered to by all responsible parties and carry out disciplinary action for non-compliance as appropriate.
13. Ensure adherence to the Corporate Integrity Program and the Performance Expectations at all times.
14. Perform other related duties as directed by the Administrator and/or person of authority including possibly working the cart if necessary.
POSITION QUALIFICATIONS:
EDUCATION AND EXPERIENCE
Graduate of an accredited Practical Nurse or Vocational Nursing Program or Registered Nurse program. 2 years nursing experience preferred. Previous experience in a Clinical Liaison/Marketing role for a long term care facility required. Presents a professional and organized approach to promoting the facility to community referral sources and professionals required. Ability to
communicate and relay both clinical and marketing related information to relevant parties.
LICENSURE, REGISTRATION, COMPLIANCE CHECKS AND/OR CERTIFICATION
Certification as an Activities Professional preferred. Successfully complete &/or comply with all required background checks to include but not limited to: Ohio BCI&I fingerprint analysis, FBI (if applicable) fingerprint analysis, Nurse Aide Registry, HHS/OIG list of Excluded Individuals/Entities (LEIE) & Ohio Medicaid Provider Exclusion and Suspension List.
Valid LPN or RN license in the State of Ohio. Valid driver’s license and proof of current insurance and dependable transportation required. CPR certification required.
RESPONSIBILITY FOR PATIENT CARE
Provide services based on physician orders. Communicate physician, patient and family needs with the appropriate caregivers.
COMPUTER
Intermediate to Advanced personal computer skills including electronic mail, routine database activity and word processing.
LEADERSHIP RESPONSIBILITIES
Ability to coordinate the marketing initiatives for the facility to all relevant parties within and outside of the assigned facility.
RESPONSIBILITY FOR CONFIDENTIAL MATTERS
Works regularly with some information such as medical records or reports, financial reports, information on computer screens or other electronic devices, and records that are restricted to specific persons. Must comply with confidentiality policy.
