JOB SUMMARY

Assist the Nurse Management Team in supporting Health Center nursing operations to ensure the delivery of high quality and proficient care to residents, in accordance with federal and state regulations.

ESSENTIAL FUNCTIONS OF LPN NURSE CARE PARTNER

  • Under the guidance and direction of the Nurse Management Team, assist in leading and managing Nurse Care Partners (RNs and LPNs) and Care Partners (CNAs and CMAs) to assure the desired level of resident service, care, and satisfaction. Assist in monitoring and evaluating team member performance and provide coaching, training, and corrective actions, as required.
  • Oversee, monitor, and support Montereau wound protocol to include: auditing weekly skin assessments for accuracy and completion, conducting weekly wound rounds with wound specialist, implementing wound specialist recommendations, assessing new wounds and providing nursing team members with accurate treatment orders, documenting all required information according to state and federal regulations, and communicating all wounds status at monthly risk assessment meeting, or as needed. Provide guidance and direction to nursing team members regarding treatment orders and preventive measures to be implemented, as needed.
  • Monitor all admission documentation for accuracy and completeness and routinely assist Nurse Care Partners with admissions and discharges. With the oversight of the Director of Nursing and Assistant Director of Nursing - Chateau, communicate with nursing team members regarding incomplete admission documentation and give the direction for this to be completed.
  • Partner with Health Services Interdisciplinary Team (IDT) in the evaluation of resident care operations and affect changes as necessary to improve programs and assure compliance with regulatory requirements.
  • Participate in the Risk Assessment Committee by developing written and oral reports and recommendations for the Director of Nursing concerning the operation of the Quality Assurance and Performance Improvement (QAPI) Program and collaborate with the Committee to determine appropriate preventative measures.
  • With oversight from the Nurse Management Team, assist in the investigation of resident incidents/accidents resulting in injury and complete state reportable, as indicated, for resident injury.

OTHER FUNCTIONS OF LPN NURSE CARE PARTNER

  • While working Nurse Care Partner shifts, participate in the Health Center Nurse Manager On-Call rotation to support Nurse Care Partners with critical resident care decisions and assure adequate staffing and scheduling.
  • Participate in team member meetings for licensed and non-licensed team members.
  • Assist in providing a high quality orientation to new hire nursing team members.
  • Work care base shifts, as needed, to assure proper resident care is provided.
  • Perform other related functions as required or as assigned.

LPN NURSE CARE PARTNER QUALIFICATIONS

Experience:

  • A minimum of 2 years of skilled nursing experience within a long-term care (LTC), assisted living, or hospital environment is preferred
  • Previous experience working with federal, state, and local nursing services regulations and practical experience analyzing and interpreting statistical reports while working in an interdisciplinary team environment is preferred
  • Previous experience within a Life Plan Community is preferred
  • Previous leadership experience in a healthcare setting is preferred

Education/Certifications/Licenses:

  • Current State of Oklahoma LPN license is required
  • Current CPR and First Aid certification is required

Job Specific Knowledge, Skills, and Abilities

  • Strong understanding of state and federal regulations relating to LTC
  • Strong knowledge related to establishing and following care plans
  • Strong resident care documentation skills
  • MDS knowledge preferred
  • Working knowledge of Microsoft Office programs and ability to learn new software systems (i.e. MatrixCare)
  • Solid problem solving and decision making skills
  • Strong communication skills to effectively and empathetically address critical health and death issues with a diverse resident population and family members
  • Strong interpersonal skills and understanding of quality customer management and service
  • Ability to effectively interact with a diverse leadership team, team members, residents, and third-party associates, including vendors, providers, and visitors
  • Ability to effectively manage sensitive and confidential communications and information, while maintaining a high level of poise, tact, and diplomacy

Physical Demands:

  • An extensive amount of walking, standing, and sitting, with a moderate amount of bending at the waist, kneeling, twisting/turning, reaching above the head and shoulders, and eye-hand coordination
  • Ability to routinely lift 35 pounds and push wheelchairs with loads of up to 300 pounds
  • Frequent use of ordinary communication, including normal levels of hearing and vision, including both near and far seeing, color discrimination, field of vision, and depth perception
  • Ability to use a computer or other office equipment for an extensive period of time

Mental and Emotional Requirements:

  • Understanding and carrying out oral and written instructions and reviewing/preparing detailed reports, schedules and medical records, using measuring devices, and simple calculations
  • Planning own work activities and occasionally directing and training others in their work assignments
  • Working independently or as a member of a team, and the ability to work with limited supervision
  • Routinely working on multiple tasks at the same time, under moderate amounts of pressure for results, with frequently changing priorities and requirements for extreme accuracy
