We have a fantastic opportunity for Full-Time Case Manager at Montefiore Mount Vernon Hospital.
JOB SUMMARY
This is an entry level professional position without supervisory duties. Under the supervision of the Program Manager, the case manager focuses on improving the quality of life of people living with Pervasive Behavioral Health, by ensuring receipt of services such as; entitlements, benefits, housing, transportation, legal, community/social programs. The Case Manager will assist patient to overcome barriers to Health & Wellness. The Case Manager’s responsibilities involve both on site and community service delivery.
The Case Manager will be expected to provide a variety of services such as;
POSITION QUALIFICATION:
An associate’s degree in Health and Human Services, Social Work, Psychology, Liberal Arts required and one year minimum work experience with individuals diagnosed with mental health issues.
Bachelor’s Degree in above disciplines can substitute for one year of experience.
Individual must possess good verbal, writing and computer skills.
Bilingual (Spanish/English) preferred.
Valid NYS Driver's license
CASAC license required
SKILLS REQUIRED:
This position requires good interpersonal and organizational skills, with excellent reading comprehension, writing, and time management skills. Computer literacy is required including ability to use Microsoft Word & Outlook.
