JOB SUMMARY: Facility is looking to hire a Social Worker who will also run the Admissions department. 

Experience, qualifications, knowledge: Must be a CSW and hold the certificate for same. Experience with the elderly and a sensitivity to their special needs. The Social Worker must be a part of the interdisciplinary team and meet the established standards in the NJ and Federal Regulations. Be easy going and have warm relationships with families. Know the process of discharge planning, charting and documenting. Prior nursing home experience a must.  

Will also oversee running the Admissions department. Keeping track of referrals and dealing with hospital case managers. Knowledge of Medicare, Medicaid and HMO insurance process. Ensuring a smooth admission process. Willing to train the Admissions department part of the job.
