Monadnock Family Services is Hiring a Case Manager - Health & Wellness Specialist Near Keene, NH

Description

A Health and Wellness Specialist Case Manager will provide nutritional assessments, evaluate diet and food intake, digestive issues, and grocery shopping habits, and makes nutritional recommendations for balancing the body and promoting optimal wellness. The specialist will collaborate with psychiatric and medical services as, well as client’s treatment team. The Specialist will provide client education around healthy eating and lifestyle choices in individual or group settings. In addition, the Specialist will perform all duties of a case manager, coordinating a wide variety of therapeutic, educational, basic supports and integrative services to psychiatrically disabled children to ensure they receive the services and assistance required for successful maintenance in the least restrictive environment and for continuity of care. Direct intervention in the community setting is provided to enhance goal attainment. Hours vary through the week to accommodate families/clients. Qualifications: Bachelor’s Degree in a human service field, along with a degree, certificate or concentration in health, nutrition or wellness. At least one year experience working with SED children and their families; skills in advocacy, outreach, assessment, crisis intervention and supportive counseling; and strong communication skills. Good driving record required.

Requirements

1. Bachelor’s Degree in psychology social work, psychosocial rehabilitation or related human services field

-or- 

Associate’s degree in related field and two years’ experience working with individuals with mental illness.

-and- 

A degree, certificate or concentration in health, nutrition or wellness.

2. Understanding of mental illness and desire to reduce stigma and effectively support this population. 

3. Skills in advocacy, outreach, assessment, crisis intervention and supportive counseling.

4. Strong communication and organization skills.
