JOB TITLE: Clinical Dietitian

JOB SUMMARY
Utilizing the nutrition care process, Clinical (Registered) Dietitians will provide nutritional care to individual patients through assessment of nutritional status, identifying of specific nutrition problem (nutrition diagnosis), implementing appropriate nutrition intervention, and monitoring outcomes directly related to the nutrition diagnosis in accordance with provider orders, regulatory agencies, Mon Health System policies, and department guidelines. Clinical Dietitians will act as a liaison between patients, providers and Nutrition Services. Clinical Dietitians will mentor and assist Dietetic Interns, Diet Clerks, Hostesses, and Food Service Employees to promote greater patient tolerance to therapeutic diets ordered by providers. Clinical Dietitians will fulfill their duties to ensure continuity and quality of care which fosters well-being and is in the best interest of the patient and their families.

Experience/Physical Requirements:
3-5 years. Previous supervisory experience preferred. Must possess working knowledge of Cardio & Pulmonary Rehab Practices.

JOB SPECIFIC RESPONSIBILITIES
Nutrition Assessment Evaluates weight changes. Assesses nutrition needs using established practice guidelines. Addresses need for diet education on all patients. Completes assessments within established timeframes. Evaluates health and disease conditions for nutrition-related consequences Evaluates physical findings. Evaluates medication management. Evaluates diagnostic tests and procedures. Evaluates physical activity habits and restrictions. Evaluates psychosocial, socioeconomic, functional, and behavior factors related to food access, selection, preparation, and understanding of health condition. Nutrition Diagnosis Uses standardized language from International Dietetics and Nutrition Terminology. Derives nutrition diagnosis from the assessment data. Determines etiology based on nutrition related cause and/or contributing risk factors. Clusters signs and symptoms based on defining characteristics of nutrition problem. Documents nutrition diagnosis (es) in a written statement: includes problem, etiology, signs and symptoms. Re-evaluates and revises nutrition diagnoses when additional assessment data becomes available. Nutrition Intervention Selects specific intervention strategies that stem directly from the Problem Etiology Signs/Symptoms statement. Utilizes nationally developed evidence-based practice guidelines to determine the intervention. Determines client-focused expected outcomes for each nutrition diagnosis (problem). Outcomes are individualized, clear, measurable, and observable. Communicates intervention. Addresses learning needs, preferences, readiness to learn, understanding and expected compliance with nutrition education. Nutrition Monitoring and Evaluation Monitors patient’s progress. Follow-up occurs within established timeframes. Measures outcomes that are standardized, evidence-based outcome indicators. Evaluates and addresses progress toward goals and revises goals as new nutrition problems arise. Performs Other Duties as Required Attends mandatory in-services, staff meetings, and actively participates in self-education. Completes all required competency assessments on time. Maintains required job knowledge (e.g. licensure and credentials, continuing education etc.). Assists in achieving compliance with Joint Commission standards. Assists in policy revisions related to patient nutrition care. Maintains productivity standards and practices effective time management and prioritizing of tasks. Supports and promotes the organizations' infection control, safety, risk management and customer/guest relations programs. Performs Other Duties as Required.
