Provide comprehensive health care and services in a team oriented primary care setting with responsibility for examination, referral and treatment of patients.


Summary:

The Behavioral Health Care Manager functions as a core member of a Collaborative Care team that involves the patient's Primary Care Provider, a Psychiatric Consultant, and, when available, other mental health providers in the primary care setting.

The BH Care Manager is responsible for coordinating and supporting mental health care within the clinic and assist in facilitating referrals to clinically indicated services outside the clinic.

No supervisory responsibilities are required for this position


Essential Functions and Duties:

Support and closely coordinate mental health care with the patient's primary care provider and, when appropriate, other mental health providers.

Screen and gather information about common mental health and substance use disorders.

Provide the patient with information about common mental health and substance use disorders and available treatment options.

Use measurement-based tools to track patient (in person or by telephone) for changes in clinical symptoms and gather information about treatment side effects or complications: cue other members of the treatment team as needed to address these issues.

Support psychotropic medication management prescribed by PCPs by asking questions about treatment adherence, side effects and other complications, and effectiveness of treatment; cue other member of the treatment team if interventions are needed.

Discuss with patients their current activities, discuss how being more active improves depression and plan additional activities with patients.

Facilitate in-clinic or outside referrals to evidence-based psychosocial treatments (e.g., CBT, IPT) as directed by other team members. Follow up that patient has connected and attended.

Participate in regularly scheduled (usually weekly) caseload consultation with the psychiatric consultant. Facilitate communication regarding treatment recommendations to the patient's PCP. Consultation will focus on patients new to treatment or who are not improving as expected, as evidenced by PHQ-9 scores.

Support patient engagement and follow-up in care.

Document patient follow-up and clinical outcomes using a registry. Document in-person and telephone encounters in the registry and use the system to identify and re-engage patient in Collaborative Care.

Document patient progress and treatment recommendations in the registry so they can be easily shared with PCP's, the psychiatric consultant, and other treating providers. Documentation into the official medical record must be done by a licensed provider for whom it's with their scope of practice.


Requirements

Required Skills/Abilities:

Experience with screening for common mental health and substance use disorders.

Experience working within a Primary Care or Health related setting.

Experience working with patients who have co-occurring mental health,ubstance use, and physical health problems.

Ability to prioritize and delegate multiple tasks and projects to ensure each is completed correctly and in a timely manner.

Proficient in Microsoft Office.

Proficient in utilizing Electronic Health Records.

Able to establish and maintain effective and collaborative working relationships and demonstrates compassion, respect, courtesy, and tact when interacting with others.

Actively listen, elicit needed information, and communicate information effectively and accurately.

Make appropriate and timely decisions in stressful situations and maintain objectivity.

Ability to maintain confidentiality.

Complete 12 continuing education credits annually. Also, completion of internal credentialing which includes BCLS, Infection Control, and Child Abuse Training.


Education and Experience:

Valid New York State Clinical License, LMSW

Master's degree in social work

Possess an active CPR certification

Effective time management and excellent organization and planning skills

Ability to communicate effectively in writing and verbally to internal and external stakeholders

Bilingual English with Spanish preferred.


Physical Requirements:

Prolonged periods sitting at a desk and working on a computer.

Must be able to lift to 15 pounds at times.

Must be able to traverse all facilities in all types of weather.
