GENERAL SUMMARY:

Reporting to the Community Programs Manager and as a member of the Information Center, the Community Care Coordinator’s role is to educate and empower older adults, persons with disabilities, and their caregivers to be in control of their personal options, planning, and decisions for long-term care and services by providing resources and education through our Options Counseling Program.

ESSENTIAL FUNCTIONS:

  • Provide unbiased information, resource counseling, referrals, education, training, and support to older adults, persons with disabilities, and their caregivers using a person-centered approach that ensures the consumer's goals and their preferences are honored.
  • Meet with consumers in a variety of settings including their home, rehab, the hospital, group medical practice office, Minuteman office or other location of their choice.
  • Represent Minuteman through outreach efforts and other meetings, community events and partnerships, as needed, at the request of the Community Engagement Manager.
  • Educate the community by offering group presentations using pre-existing or newly created curricula. 
  • Work as team member with staff from other Minuteman programs to coordinate care planning and resources for consumers.
  • Input data for measurable outcomes and to meet documentation regulatory guidelines and reporting requirements as required by EOEA as well as provide updates, as requested, to the Executive Director, and other agency staff and Board as needed.
  • Participate in ongoing professional training and supervision to maintain and enhance skills to best meet the needs of consumers and their families.

Communication Skills: Listens, speaks, and writes clearly and concisely; ensures all involved are kept informed about developments and plans; shares ideas and information with others who might find them useful; keeps supervisor informed about progress and problems.

Critical Thinking and Problem Solving: Recognizes and accurately evaluates the signs of a problem and utilizes available resources to resolve; analyzes current procedures for possible improvements; notifies supervisor of problems in a timely manner.

Teamwork and Cooperation: Demonstrates willingness to work with others toward a common goal as opposed to working in competition with others.

Time Management: Organizes and schedules people and tasks demonstrating the ability to balance requisite number of home visits per month with maintenance of notes; regularly produces accurate, thorough, professional work.

REQUIRED EDUCATION and EXPERIENCE:

  • Bachelor’s degree required in social work or related field as well as 3 years working with older adults, caregivers, families, and persons with disabilities. Licensed Social Worker (LSW) preferred.
  • Working knowledge of community resources, entitlements and the Massachusetts Aging Services Access Point (ASAP) system is a plus.
  • Experience working as a member of a team.
  • Second language skills are welcomed.
  • Strong interpersonal and communication skills both on the phone and in person is a must.
  • Excellent presentation skills and professional demeanor and appearance required. 
  • Comfortable working in a variety of settings such as private residences, nursing facilities and rehab centers.
  • Candidate must be flexible, assertive, and a motivated, organized self-starter.
  • Working knowledge of Outlook, Word, Excel and PowerPoint required.
  • Comfortable accessing and entering information into a complex database. 
  • A reliable car, current driver’s license and safe driving record required.
