Minnesota Assistance Council for Veterans is Hiring a Remote Justice Involved Case Manager

The Justice Involved Veterans Case Manager will utilize a network of internal and external services to assist Veterans that are currently incarcerated statewide, so they can release from correctional institutions into housing stability. Providing face-to-face outreach to Veterans is essential to developing rapport, increasing engagement, and identifying Veterans in need of MACV assistance. The Justice Involved Veterans Outreach Coordinator shall perform these responsibilities that contribute to the MACV mission to End Veteran Homelessness in Minnesota, and to do so in ways that aspire to programmatic and administrative excellence.

Primary Duties and Responsibilities

  • Provide in-person, statewide, outreach to Justice Involved Veterans incarcerated within the Minnesota Department of Corrections (DOC) and Metro County detention centers/jails on a monthly, bi-monthly, or as needed basis.
  • Must be willing to travel to all Minnesota Correctional Facilities to include the Department of Corrections and various County Jails.
  • Must be able to pass a background check with both the Department of Corrections and County Detention Centers to enter the facilities.
  • Provide education on MACV programs and services available to Veterans incarcerated in state and county correctional facilities.
  • Attend Transitions Fairs, Career Fairs, and other events hosted by the Minnesota Department of Corrections as scheduled on behalf of MACV.
  • Develop a pre-release plan with the Veteran/household and with state and county correctional facility case manager which includes short-term and long-term goals with objectives to develop pathways to housing stability and prevent reincarceration.
  • Develop and distribute a needs assessment; collect results and analyze gaps in services to be addressed for those exiting state and county correctional facilities into homelessness.
  • Perform client intakes that include required documentation for admission, referral needs, explanation of program and expectation. Upload required documentation and entering client information and case notes into the Salesforce client management/tracking system.
  • Identify appropriate housing options immediately upon release including coordinating Transitional Housing interviews and implement the discharge into MACV or community transitional housing programs.
  • Engage with Veterans Administration Justice Outreach, MDVA Justice Outreach, Veterans Treatment Court, and other Veterans Service organizations throughout the state.
  • Conduct outreach and develop partnerships with housing and treatment programs statewide that will accept Veterans upon release from incarceration.
  • Connect Veterans to the Justice Involved Veterans Case Manager on cases within their scope to oversee the ongoing post-release supports.
  • Participate in justice involved summits and trainings offered by community organizations and the Department of Corrections.
  • Perform other duties and administrative tasks as assigned.

Desired Skills and Other Characteristics

  • Bachelor’s degree or higher with an emphasis on restorative justice, Second Chance initiatives, Social Work, or knowledge of corrections, re-entry services, or a combination of relative education and experience
  • Experience and knowledge of judicial, correctional, and re-entry processes from a restorative justice standpoint, and Veteran services within the State of Minnesota.
  • A broad understanding of the barriers involved when a Veteran reenters the community from incarceration.
  • Demonstrate ability to collaborate with others effectively; to manage conflict; to participate as a team member; and to give and accept criticism constructively.
  • Ability to produce effective case notes and communicate verbally and in writing with outside agencies.
  • Have a high regard and ability to meet schedules and timelines; demonstrate excellent ability to work independently and in group settings.
  • Share MACV’s social justice vision to serve high barrier Veterans and to provide service in a way that aspires to standards of administrative and programmatic excellence.
