RN Nursing Supervisor – Personal Care Agency

Contract Part-time Work - Hours each week anywhere from 0 up to 20 hours per week

**Work week will fluctuate due to the nature of the work load**

Rate of Pay - PER DIEM CONTRACT

Duties of Position:

Seeking an RN to travel within and outside of Green Bay to service our clients

The RN Nursing Supervisor is responsible for all actions that are related to the assigned home visits by overseeing the work of the personal care worker and the primary needs of the clients such as assessment of the clients and their plan of care through planning, coordination, and implementation and evaluation of the personal care provided to them. They also have to maintain the quality of client care/services in accordance with the agency’s objectives and policies.

Position Responsibilities:

• Performs all scheduled visits per company policy at the client’s home every 50 to 60 days to assess or re-assess the client’s medical condition and making sure the PCW (personal care worker) is following the client’s active POC (plan of care) and understanding their obligations as a PCW caring for the client as their supervisor.
• Develop a POC (plan of care) for a new client after the initial home visit assessment and consulting with the client and/or family member giving full consideration to the client’s preferences for service arrangements and choice of personal care workers (PCW), interpret the POC to the PCW, include a copy of the POC in the client’s record and review the plan at least every 60 days and update it as necessary.
• Assess the client’s medical condition and needs accordingly based on their written POC (plan of care) and determine the appropriate care plan based on the client’s current assessment physically, psychologically, socially, and environmentally and make the necessary updates to their POC (plan of care) as needed. Determine the level of care that is appropriate for the client based on the client’s diagnoses.
• Notify the client’s physician as soon as possible of any significant changes to the client’s condition during the home visit or based on reports from PCW or the clients themselves.
• Appraises staff performance during their probationary period and annually by providing them the teaching and counseling that is necessary to enhance the delivery of services and the development of direct care staff.
• Train new PCWs on properly caring for the clients by demonstrating to the PCW how to perform the tasks and services needed by the clients as well as training current PCWs with on-going evaluation as necessary.
• Evaluate the competency of the personal care worker’s (PCW) to perform the services which occurs on hire and at least one the every 60 day supervisory visit.
• Review, verify, and sign all PCW weekly timesheets and schedule submitted for changes or updates and file into PCW files.
• Ensures all pertinent paperwork is put in the client’s record and completed in a timely manner.
• Ensures the agency’s continuing education program i.e.- in-service education and continuing training.
• Performs other tasks as designated by the agency administrator not listed here.
• RN will have to travel outside of the Green Bay area to perform home care assessment/supervisory visits for our clients located outside this area such as Appleton, Oshkosh, Wausau, Manitowoc, Sheboygan, Milwaukee, and Madison area. Paid mileage reimbursement.

Minimum Education and Qualifications:

Graduated from an accredited School of Nursing

Must currently be licensed by the State of Wisconsin as a Registered Nurse under s.441.06 Wisconsin Stats

Training and experience in the provision of personal care services or a related program

At least one year of supervisory experience in Hospice, CBRF, Home Health, or Personal Care Agency (PCA)

A valid Wisconsin Driver’s License and reliable transportation with current auto insurance for home visits traveling at clients’ home

Must be willing to undergo a criminal background check

Must be willing to travel for new client assessments or client renewal/supervisory visit when needed in the Greater Green Bay area, the Fox Valley area, Wausau, Madison, Milwaukee, Sheboygan, Manitowoc, etc....

• This job is a per diem contract job. Per diem is where you are paid a set amount once the job is completed and/or approved. Each week workload is not the same as it fluctuates based on the deadlines for each client we service. You do have flexibility to work. Will you be okay with this kind of work?
