Midlands Choice Home Care, LLC looking for a Registered Nurses to provide case management and care for our clients and patients in their home environment. Our Registered Nurses will be responsible for educating our clients, their families and our Home Aides/CNA's on prevention and healthy habits. The ideal candidate must be a responsible and a well-trained professional who is dedicated to improving each client's quality of care.

Starting Salary: $50.00 to $65.00 per assessment (depending upon level of experience). Additional pay will be discussed upon hire for trainings, workshops, etc.

Responsibilities:

• Monitor patient’s condition and assess their needs to provide the best possible care and advice.
• Assess potential patients and clients for non-skilled home care services and generate a plan of care for CNA's and Home Aides.
• Provide timely documentation.
• Provide timely re-assessments for continuation of patient client care.
• Provide instant medical care in emergencies.
• Supervise, train and co-sign LPNs and nursing assistants' documentation.
• Foster a supportive and compassionate environment to care for patients and their families.
• Expand knowledge and capabilities by attending educational workshops, conferences etc. (Additional pay will be discussed upon hire).

Skills:

• Proven experience as a Registered Nurse.
• Excellent knowledge of nursing care methods and procedures.
• Excellent knowledge of emergency care.
• In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination etc.) and willingness to follow them at all times
• Excellent communication and interpersonal skills.
• Excellent problem solving, time management and organizational skills.
• BS or other diploma from a Nursing Program.

Preferred Experience: Home Care/ Home Health Care
