Senior Nursing Director - Care Coordination / Full Time / 40 Hours

GENERAL OVERVIEW

Brigham and Women's Hospital is seeking a Senior Nursing Director, an outstanding leader, to lead all aspects of Nursing Care Continuum Management for BWH and BWFH.

As a leader of the Nursing Care Continuum Management (CCM) division, the role reports to the SeniorVicePresidentPatientCareServices, ChiefNursingOfficer,theDeputyChief Nursing Officer/VP Nursing, and Vice President of Clinical Operations and Care Continuum for practice professional standards, quality, and safety. With 24-hour accountability the Senior Nurse Director (SND) in collaboration with the service, team, and departmental colleagues, assures that competent, compassionate patient care is consistently provided for patients and families in inpatient, outpatient, and community settings within the division. They participate in the development, refinement, communication, and actualization of the BWH and BWFH Department of Nursing (DON) vision, professional practice model, and relationship-based care delivery system. The SND is responsible for nursing practice, quality measurement and improvement, education and professional development, clinical information systems, fiscal oversight/planning and human resources related matters, patient care within the division of Care Continuum Management leadership development, and strategic planning. The SND of Care Continuum Management, is responsible and accountable for the overall clinical and administrative leadership of RN Care Coordination (RNCC). This role will directly supervise and manages staff within Nurse Care Coordination. The scope of responsibility includes professional nursing care coordination practice, human resource management, educational development, research, continuous improvement, utilization management, DRG program and MGB, Brigham (BWH and BWFH) network initiatives. This position will work in partnership with the executive director of post-acute strategy and complex care, senior medical director of clinical operations and executive administrator of care continuum management.

Principal Duties and Responsibilities

B. Knowledge
  1. Has significant clinical and management experience related to nurse CCM, particularly supporting workflows, critical issues, and staff development.
  2. Has experience of case management standard processes and optimized workflows.
  3. Participates in formulating the department's, Patient Care Services and the organization's vision and strategic plan.
  4. Communicates and operationalizes the strategic plan and can link the plan to tangible outcomes.
  5. Uses management information to determine trends, forecasts, and marketing strategies, which have implication for the institution's operational and strategic direction.
  6. Develops and maintains an operational plan based on the Hospital and DON mission and strategies.
  7. Develops and takes part in creating systems to monitor the patient care environment, knowledgeable about and mindful of relevant regulatory compliance. Including environment of care plan to ensure a safe, infection-free, and efficient area for
  8. patients, visitors, and employees.
  9. Assures regulatory compliance through leader and staff education and participation in continual readiness initiatives/ programs.
  10. Assures development and implementation of educational and professional development programs, which meet the needs of leaders and staff.
  11. Provides leadership support for and participates in the expansion of nursing and other clinical research programs.
  12. Applies research findings in developing practice standards and guidelines.
C. Excellence
  1. Sets RNCC practice standards, monitors and evaluates the quality-of-care coordination delivered in accordance with established Departmental, Professional, and Regulatory standards. Ensures ongoing sustainability with process improvement projects.
  2. Considers opportunities for improvements in clinical workflows, potential efficiencies, and guides RNCCs in improvement activities.
  3. Develops and sustains a collaborative environment that is patient-focused and supports excellence in care coordination and interdisciplinary care.
  4. Facilitates communication of information between staff, administration, clinical services, and departments. Ensures regular communication with department, service-line leadership to receive and respond to feedback bi-directionally.
  5. Identify trends in patient progression opportunities and raises to leadership.
  6. Participates in institutional, community, state, national, and/or professional organizations concerned with care coordination and health care issues.
  7. Collaborates with the Professional Development Manager for CCM to identify learning needs of staff and provide for continual learning and development of staff to meet innovative practice standards.
  8. Hold regular cadence of staff meetings to receive and respond to feedback.
  9. Hold regular meetings with department/service-lines related to case management concerns.
  10. Works in collaboration with the Executive Director of Post-Acute Strategy and Complex Care related to appropriate critical issues to complex care team and identifies learning opportunities for RNCCs for complex patients.
  11. Establishes and evaluates annual departmental goals in conjunction with other departments.
  12. Contributes to development and integration initiatives.
  13. Demonstrates and promotes a professional practice environment that recognizes excellence in clinical nursing practice; participatory decision making around practice issues and the work environment; research and theory-based practice; care delivery systems and human resource development and management.
  14. Provides input into development and implementation of information systems to support patient care. Provides formal and informal opportunities for feedback regarding professional development to leaders and staff and to develop personal professional development goals.
  15. Accountable for contributions to planning, implementing, and evaluating the DON Quality Plan, inclusive of nurse sensitive indicators and patient satisfaction metrics.
  16. Supports and emulates an environment focused on quality and safety with a just culture. Participates in review process and improvement measures.
  17. Actively participates in the quality improvement programs, assuring clear articulation of the quality agenda by all staff in their patient care area(s) and/or specialty patient populations.
  18. Develops, revises, or reviews all policies and procedures related to service area. Has intimate knowledge of policies and procedures and monitors staff compliance.
  19. Organizes and directs workflow, solves intra and interdepartmental problems that inhibit workflow and quality, motivates employees to optimal performance, and acts as a resource to staff for clinical, ethical, or system issues.
D. Collaboration
  1. Collaborates with the executive director of post-acute strategy and complex care, senior medical director of clinical operations and executive administrator of care continuum management to support entire care continuum management team.
  2. Represents the team of nursing with senior management, department administrators, and medical leadership.
  3. Actively builds inter-professional relationships to assure dialogue around patient care and issues to enhance outcome driven patient care.
  4. Participates in and works jointly with the Nurse Executive Board.
  5. Seeks resources/acts as a liaison for nurse leaders within the designated departments/division.
  6. Collaborates with Human Resource leaders/staff re: personnel management as appropriate.
  7. Collaborates in division wide recruitment and retention strategies and planning.
  8. Represents DON and provides leadership on various hospital and nursing committees.
  9. Collaborates with colleagues on clinical research studies.
  10. Administrator on call duties approximately every 6 to 8 weeks to address issues raised from providers, RN's or outside agencies during evening and off-shift hours, including weekends.
E. Relationship Based Care
  1. In partnership with the Assistant Nurse Director, Professional Development Manager and Director of Utilization Management supports the RNCCs with development, day-to-day management and critical issues.
  2. Interviews and hires department nursing personnel considering hospital goals, departmental needs clinical expertise, team temperament, diversity, and in accordance with collective bargaining agreement.
  3. Assures that all employees are oriented to the Hospital, Department, and assigned Clinical Service and that appropriate documentation of competency is completed.
  4. Assures timely completion of competency evaluation and performance appraisal in collaboration with Professional Development Manager.
  5. Creates an environment that celebrates the professional growth of nursing practice.
  6. Responsible for creating and maintaining a caring, therapeutic, healing, patient- and family-centered care environment.
  7. Assures the presence of interventions to promote/assure a patient and family centered healing environment.
  8. Assures appropriate staffing plans and budget to meet patient care needs on a continuous basis.
  9. Promotes an inclusive environment supportive of a patient- and family- centered care for a diverse community.
F. Financial Planning and Management
  1. Contributes to the preparation and monitoring of personnel, supply, and capital budgets for Care Continuum Management in collaboration with Administrative Director.
  2. Collects and analyzes data regarding financial plan variances and develops plan to correct or makes sound proposal for adjustments to financial plan.
  3. Assesses the need for adjustment for resource allocation based on changes in patient populations, clinical programs, length of stay, costs of care and payer regulations.
  4. Educates staff regarding department and hospital budgets in order to promote awareness of individual accountability and responsibility for department fiscal management.
  5. Seeks staff input and makes recommendations for system and process changes to support improvement efforts.
  6. Uses hospital and service-line outcome data to assess and improve care coordination performance.
  7. Determines resources that are needed to meet patient care and quality of work-life goals in joint effort with the clinical and administrative leadership team.
  8. Collaborates with inter-professional team in developing financial plans for new and existing programs.
  9. Provides leadership to clinical and nursing management staff in the development and management of capital and operational budgets.
  10. Allocates and monitors resources to achieve productivity, customer service and staff development goals.
  11. Participates in determining and managing nursing resource requirements and plays an active role in recruitment, retention, and growth of programs that meet these requirements.
QualificationsQualifications
  1. Current registration in nursing in the Commonwealth of Massachusetts.
  2. MSN (or) DNP (or) PhD, (or) a BSN with a Master's degree in a related field, required.
  3. Current certification in case management or nursing administration is required or to be attained within two-year period from date of hire. Certification must be maintained while employed in this position.
  4. Minimum of 3-5 years' experience in nursing administration within an academic medical center preferred.
Skills and Abilities Required
  • Must have demonstrated ability in management of diverse activities pertaining to patient care service line or clinical competence in nursing practice.
  • Expert interpersonal and communication skills.
  • Must possess analytical abilities necessary to organize, to supervise and to evaluate the work of others; to develop and to administer policies, procedures, budgets, and use current concepts of nursing practice.

Supervisory Responsibilities:Position may include supervision of nursing directors, assistant nursing directors, nurse/flow/operations managers, professional development managers, and other levels and roles of technical and support personnel.

Accountability:Reportstothe DeputyChiefNursingOfficer/VPofNursing and VP of Clinical Operations and Care Continuum Management foroperationalelements.Reportsto the Senior Vice President and Chief Nursing Officer for quality and professional development.

Working Conditions:Worksin a variety ofpatient care environments throughout the institution. Will require visits to patient care units and to community agencies. This includes travel to off campus locations such as Chestnut Hill, Foxborough, and other current and future Brigham (BWH and BWFH) licensed and/or affiliated practices with procedural service offerings.
