Alongside clinical care, researchers and teachers from Metrocare’s Altshuler Center for Education & Research are advancing mental health beyond Dallas County while providing critical workforce to the state.

The CCBHC Care Coordination Manager III will be responsible for assisting with the supervision of the CCBHC Care Coordination Team, including both Definitive and Enhanced Care Coordinators.

The Definitive and Enhanced Care Coordinators organize access to necessary services and build a therapeutic team to include natural supports and providers for those who are participating in mental health and substance use treatment. The CCBHC Care Coordinators use knowledge of social drivers of health, diagnostic characteristics, symptomology of primary mental illness and knowledge of medications, side effects and benefits to provide services in a person-centered, supportive, community-based environment. The Coordinators are primarily responsible for organizing patient care activities and sharing information among all of the participants concerned with an individual in services care to achieve safer and more effective care. This position depends upon successful relationship building with community partners and referral sources to ensure individual in-service access to the right care at the right time. The CCBHC CC Manager is monitored by periodic supervision.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

The essential functions listed here are representative of those that must be met to successfully perform the job.

  • Assisting with clinical and administrative supervision of Care Coordination Team to include hiring, training, setting productivity expectations, monitoring for performance, conducting annual evaluations, and participating in adherence to budgetary expectations.
  • Receive Care Coordination Referrals, review for complexity and assign to the appropriate staff.
  • Interface routinely with the RN Care Coordination Manager, CMIII, TL Manager, and other site managers to ensure the program operates at maximum capacity and efficiency.
  • Routinely utilizes advanced communication skills both written and verbal, and problem solves daily.
  • Assists with achieving monthly, targeted budget goals through effective management of revenue streams and controlling variable expenses in collaboration with the RN Care Coordination Manager.
  • Assists with completion of weekly reporting according to Continuous Operations Improvement guidelines and is pro-active in developing solutions around exceptions.
  • Assists with ensuring every program and team complies and meets agency performance metrics, e.g., volume, late charting, unauthorized services, productivity, and training.
  • Assists with ensuring the team(s) achieves targeted clinical outcomes (keeping people out of the hospital, helping people find and keep jobs, stable housing and adhering to medication appointments.
  • Assistance with scheduling and completing all internal and external provider appointments
  • Interacting with other team members to provide comprehensive and timely assistance in resolving issues that are barriers to the consumer receiving services
  • Coordination with internal and external providers to ensure a holistic approach to care that considers all areas of an individuals life, the individuals wants and goals and all treatment interventions
  • Documenting services in the electronic health record within 24 hours after services have occurred
  • Assists the individual in service in developing and/or strengthening natural supports who will participate in care coordination activities
  • Helping to maintain a list of community resources for successful external referrals
  • Maintains confidentiality of information concerning consumers and family members
  • Serve as care coordination mentor for care coordination staff.
  • Conduct job responsibilities in accordance with the ethical standards of conduct, state contract, appropriate professional standards and applicable state/federal laws
  • Keeps the RN Care Coordination Manager aware of all issues that confront any team and/or program
  • Create and/or enhance an existing person/family-centered recovery plan that identifies an individuals goals,
  • Performs other duties as assigned.

  • Ability to assess and organize complex information regarding an individuals goals in useable format that is clear to the individual and participating members of the therapeutic team (natural supports, internal and external providers)
  • Knowledge of Social Drivers of Health and how to assist an individual in service in accessing necessary services
  • Clinical skill for evaluating accuracy of clinical screening and providing appropriate clinical intervention
  • Knowledge of Trauma Informed Recovery Planning, Harm Reduction, Motivational Interviewing and Person-Centered Approaches to Care
  • Interpersonal skills to communicate and encourage collaboration among therapeutic team members

QUALIFICATIONS

EDUCATION AND EXPERIENCE:

  • Bachelors Degree in social work required, psychology, criminal justice or related human services field
  • Minimum of 2 years experience in behavioral health care, working with individuals with severe persistent mental illness and/or substance use disorders
  • Strong knowledge base of Dallas County community providers and resources to include behavioral and physical health care, housing, transportation, employment/academic supports and other social service agencies

OR

  • Masters degree in Psychology preferred, Social Work, or related field.

MATHEMATICAL SKILLS:

  • Basic math skills required.
  • Ability to work with reports and numbers.
  • Ability to calculate moderately complex figures and amounts to accurately report activities and budgets.

REASONING ABILITY:

  • Ability to apply common sense understanding to carry out simple one or two-step instructions. Ability to deal with standardized situations with only occasional or no variables.

COMPUTER SKILLS:

  • Use computer, printer, and software programs necessary to the position (i.e., Word, Excel, Outlook, and PowerPoint).
  • Intermediate level on MS Excel, Email and word processing programs
  • Ability to utilize Internet for resources.

CERTIFICATIONS, LICENSES, REGISTRATIONS:

  • Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. If licensed in another state, must obtain Texas Driver License within three (3) months of employment.
  • Liability insurance required if employee will operate personal vehicle on Center property or for Center business. Must be insurable by Centers liability carrier if employee operates a Center vehicle or drives personal car on Center business. Must have an acceptable driving record.
  • TB Test, CPR, First Aid and SAMA to be provided within 30 days for hire
  • TAC web-based training
  • Certified Community Behavioral Health Clinic Trainings completed within the first year of hire
