Works with patients, families, physicians, staff, community providers and managed care companies to identify and implement discharge planning services for clients admitted to the Behavioral Health Unit. Verifies benefits to connect patients with appropriate aftercare plans and entitlements, ensuring a smooth transition through the continuum of care.
Education – Associates degree in Human Service-related field or equivalent years of experience.
Experience – Minimum of 1 year working with behavioral health clients. 1-3 years of experience preferred.
Licensure/Certification – N/A
Knowledge/Skills/Abilities – Skills in communicating effectively with a variety of people of various socio-economic and educational background. Knowledge of community organizations and social services backgrounds.
