Meridian Palliative & Hospice Care Services is Hiring a Home Visiting Nurse Practitioner Near Vernon, IL

Job description

Working at Zigma Visiting Physician & Meridian Health Care you will be responsible for clinical evaluation and management of patients in a home setting, as Mid-level, providing diagnosis, treatment, consultation, and follow-up under the supervision of one physician. You will be accountable to utilize the appropriate level of care: working with Zigma Visiting Physicians, MHC and specialists to facilitate immediate treatment and coordinate care to ensure quality patient care. You will be responsible for managing the care of patients in the home setting in the same way a primary care provider is responsible for managing outpatient care. You will help to optimize care through more effective practice and accelerate the patient’s progress by ensuring efficient yet compassionate care. Nurse Practitioner/MD performs functions that require substantial specialized knowledge, judgment, and skill and must be able to make judgments accordingly.

Essential Functions:

  • Oversee and be accountable for the overall quality of care and clinical outcomes
  • Responsible for the diagnosis and treatment of acute, chronic, and long-term healthcare issues
  • Communicates with patient, patient’s family, and/or caregiver regarding treatment plan and initiates end-of-life discussions and hospice when deemed appropriate
  • Effectively identifies, evaluates and addresses disease prevention and health promotion issues of the population in the practice while administering quality patient care
  • Educates patients and/or families about preventive care, medical issues, and use of prescribed medical treatments and/or medications
  • Demonstrates, by action and attitude, the standard of high patient satisfaction, treating each patient with respect
  • Attend and participate in all regularly scheduled meetings
  • Responsible for timely documentation
  • Ensure coding and documentation activities are met for completeness, accuracy, timeliness, and reimbursement level
  • Accountable to respond to urgent needs when medically indicated
  • Attends continuing education meetings and programs as needed
  • Collaborates with other clinical and administrative staff in a positive, constructive manner
  • Meet all provider credentialing and QI standards
  • Uses protects and discloses patients’ protected health information (PHI) only in accordance with Health InsurancePortability and Accountability Act (HIPAA) standards
  • Consult with attending physician(s) as necessary

Qualifications:

We are looking for nurse practitioners that have a range of experience. Whether you are a recent graduate or a nurse practitioner with years of experience we want to work with you. You will be compensated based on experience, clinical knowledge, approach and interpersonal skills.

  • Valid/Unrestricted ARNP License
  • ANCC/AANP Board Certification
  • DEA not necessary but preferred
  • Ability to perform clinical duties within established guidelines in an organized & efficient manner
  • Ability to relate and communicate to all cultural and ethnic groups
  • Ability to collaborate as necessary to provide quality care
  • Ability to develop & maintain cooperative relationships with staff members, patients, other clinicians as a member of a multi-disciplinary team
  • Provide physical & psychosocial support to patients & their families
  • Complete & return progress notes within 24 hours of completion
  • Current license to practice in the state of Illinois
  • Certificate of completion from an accredited program

  • Able to quickly analyze, comprehend and create written/verbal clinical and business communication and documentation
  • Ability to multi-task in a fast-paced clinical setting
  • Excellent verbal, written and interpersonal communication skills
  • Ability to develop positive interaction with patients, families, physicians, administrators and teammates in order to effectively care for the patient
  • Strong technical skills, use of EMR, charting, PDFs, Word, Outlook, Google Drive (short introduction/training provided if necessary)

Job Type: Full-time

Salary: $95,000.00 - $105,000.00 per year

Hourly: $45.00-$50.00/hour

Job Types: Full-time, Part-time, PRN

Pay: $40.94 - $49.31 per hour

Expected hours: 20 – 30 per week
