Job Overview:
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. As a Registered Nurse Case Manager, you will be responsible for providing comprehensive care and coordination for patients in various healthcare settings. Essential Functions/areas of accountability

  • Reviews client referral information and responds to requests and inquiries as appropriate.
  • Collaborates with clinical supervisor regarding the eligibility and appropriateness of client for home care services.
  • Completes comprehensive assessments including OASIS date set within forty-eight (48) hours of referral for speech therapy unless othera arrangements are made
  • Determines appropriateness of referral for home care services and responds per MHHCS, Inc. policy and standard guidelines.
  • Performa initial and ongoing client assessments based on MHHCS, Inc. policy and standards of practice to ensure effective and appropriate home cares services.
  • Performs comprehensive assessments including OASIS data collection where indicated.
  • Considers the physical, physiological, and socioeconomic needs of clients, the level of family acceptance, and the ability for client needs to be met in the home

Responsibilities:
- Conduct patient assessments and develop individualized care plans
- Coordinate and communicate with healthcare providers, patients, and their families
- Monitor patient progress and make necessary adjustments to care plans
- Provide education and support to patients regarding their medical conditions and treatment options
- Collaborate with interdisciplinary teams to ensure continuity of care
- Advocate for patients' needs and rights
- Maintain accurate and up-to-date medical records
- Adhere to all relevant healthcare regulations and guidelines

Qualifications:
- Valid Registered Nurse (RN) license in the state of Illinois
- Minimum of 2years of nursing experience in primary care, acute care, or critical care settings
- Proficient in conducting patient assessments and providing appropriate interventions
- Strong knowledge of medical terminology and procedures
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Experience in behavioral health, pediatrics, or case management is preferred

Skills:
- Proficient in vital sign assessment and interpretation
- Knowledgeable in primary care, acute care, and critical care practices
- Ability to provide inhalation therapy as needed
- Strong understanding of nursing principles and practices
- Familiarity with medical terminology and documentation standards
