We are seeking a community-based, Registered Nurse Navigator to join our team in our Bethlehem, PA location.

The primary role of the Nurse Navigator is to:

* function as a health advocate to aid individuals in receiving the physical health and behavioral health care

* assist in coordinating medical and behavioral health appointments, hospital care and discharge to the community

* arranging travel to appointments

* development of personal plans for consumer health collaboration and wellness.

Earn $33.15 / Hour.

The ideal candidate will possess the following qualifications:

* Licensed as a Registered Nurse in Pennsylvania

* Currently meets all requirements per the state nursing board

* Minimum one year experience working with individuals who have mental health diagnoses

* Experience with addictive diseases and/or community mental health preferred
