● Provide transportation to Crisis Diversion Center at Wellstone Mental Health Center in

Huntsville, Alabama. (Examples of tasks: drive clients as needed to Crisis Center and

ensure clients safety.)

● Needs assessment. (Example of tasks: using required forms; determines through

assessment client's assets, deficits and basic needs; reviews needs on regular basis).

● Develops service plan. (Examples of tasks: contact provider source; write goals which

may include personal grooming and health teaching; medication knowledge; financial

aid; etc.).

● Implements plan by providing case management services. (Examples of tasks: monitor

client condition; make home visits; assist client in securing needed services).

● Represents the needs of the seriously mentally ill. (Examples of tasks: establish liaison

with other community resources; identify service gap; advocate for client).

● Participate in insuring quality of services. (Examples of tasks: participate in case

management training; complete work in accordance with standards; participate in staff

meetings; participate in peer review; participate in Total Quality Improvement task

groups.)

● Basic Living Skills Instructor. (Examples of tasks: provides didactic, process and

experiential instruction to clients).

● Meets required productivity expectations.
