Home Health Registered Nurse :
Looking for a flexible nursing career? Want to set your own hours? Menorah Park Home Health is currently seeking an RN Case Manager to cover our Cuyahoga on the west side of Cleveland, Medina, Lake and Geauga county service areas. This position gives you the flexibility to work around your schedule.
The RN, Case Manager plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. In this role, you will plan and direct services that meet the needs of individuals and families within their homes and throughout our communities.
Salary::up to $90 per visit
Essential Qualifications::

• Must have 1 year of home health experience and experience with OASIS documentation, but willing to train the right person.

• License to practice as a Registered Nurse in the State of Ohio.

• Working knowledge of the needs of the geriatric population, medically, psychologically, emotionally and socially.

• Exceptional communication skills, both oral and written.

• A team player capable of working with great independence
