Description

CULTURE:

Community Health Workers (CHWs) are members of the community who work with the local health care system. CHW's offer interpretation and translation services, provide culturally appropriate health education and information, help people get the care they need, give informal counseling and guidance on health behaviors, advocate for individual and community health needs, and provide some direct services such as first aid and blood pressure screening.The CHW will be knowledgeable of social and cultural needs and community resources for patients.

ROLE REQUIREMENTS:

  • CHW’s provide culturally appropriate health education and information
  • They can offer interpretation and translation services for healthcare and social needs
  • They often assist individuals in obtaining information and services and in navigating the complex healthcare and community organizational system when needed
  • Provide informal counseling and guidance on health behaviors, advocate for individual and community health needs
  • Work as a conduit of information between providers, medical assistants, and patients to ensure that there is a clear understanding of all information provided
  • May assist referral coordinator with referral process and other duties as assigned to promote optimal health outcomes
  • Coordinating care, providing case management support, and assisting individuals and communities in navigating health and social service systems
  • Cultural mediation among individuals, communities, and health and social service systems
  • Coaching and social support
  • Promotion for individuals and communities
  • Building individual and community capacity
  • Implementing individual and community assessments
  • Collaborate and communicate effectively with direct reports
  • Regular and predictable attendance.
  • Continuous learning in both personal and professional development.
  • Consistent review, comprehension and reply of company email and related correspondence.

CHWs’ essential functions:

  • Improved access to health care services.
  • Increased health and screening.
  • Better understanding between community members and the health and social service system.
  • Enhanced communication between community members and health providers.
  • Increased use of health care services.
  • Improved adherence to health recommendations.
  • Reduced need for emergency and specialty services.

QUALIFICATIONS:

MINIMUM:

  • Must be fluent in English and Spanish
  • Received a high school diploma or equivalent or agrees to obtain a high school diploma or equivalent.

  • Medical Interpreting Certification

MELISSA MEMORIAL HOSPITAL:

Melissa Memorial Hospital is a 15-bed critical access hospital, located in Holyoke, Colorado. While being “rural” we are equipped for any situation. It is a model rural hospital with 1 operating room, surgical procedure room, 4 bay emergency room, and radiology including 64-slice CT scanner and mammography, MRI, ECHO, and Nuclear Medicine, along with other ancillary services. The hospital is co-located with a family practice clinic and retail pharmacy services. Other services include chemotherapy, general & orthopedic surgery, podiatry, cardiology, orthopedic surgery, progressive rehab services, and wound care. MMH operates a full-time EMS service and maintains a 24/7 AirLife Helicopter base.
