Description

Position/Title: Medical Director, Board Certified Physician-East Coast

Reporting Line: Chief Medical Officer

Status: Full-Time, Exempt

About MedZed:

MedZed is the medical office of the twenty-first century, delivering care into the homes of patients with complex medical and social needs. Our multi-channel engagement approach is consistently reaching at or above 50% of members that cannot or have not been engaged through standard outreach campaigns. We utilize community health navigators, social workers and nurses in-the-home linked to MedZed primary care providers, over an intelligent member-centric telehealth platform, to deliver longitudinal team-based care that addresses social needs and health disparities while effectively managing patient outcomes. Our care model is scalable and customizable to individual client and patient needs. MedZed’s house call model has demonstrated excellent outcomes with high patient satisfaction.

Our members slip through the cracks of the traditional delivery system. They don’t regularly go to the doctor, whether due to lack of transportation, lack of trust, or language and cultural barriers. They may have access to some social services, but those are fragmented and not integrated with their medical needs. Now, more than ever, we need to provide care for them, delivered to their home. These members are often hard to find and while health plans spend substantial resources on these members, outreach efforts typically fall short.

MedZed offers an end-to end solution designed specifically for high need populations and partners with managed Medicare and Medicaid Entities.

Serving members in seven states with a team of more than 400 people and growing, MedZed has completed over 250,000 in-home visits and continues to expand throughout the country. Join the team and become part of our story.
