Social Worker - Contract Positions

(Hospital MSW)

We are seeking a compassionate and dedicated Licensed Social Worker (LSW) to join our hospital team. In this role, you will play a crucial part in evaluating the psychosocial aspects of illness and trauma, offering essential social work services to patients and families. Your primary focus will be on enhancing patients' ability to navigate treatment, cope with medical conditions, and actively engage in their care through discharge planning and case management responsibilities.

Responsibilities:

• Conduct comprehensive assessments to identify patients' psychosocial and discharge planning needs.
• Utilize knowledge of community resources to connect patients with appropriate interventions and support services.
• Educate patients and their families on care plans, available resources, and self-care strategies.
• Maintain detailed documentation of discharge planning progress and psychosocial interventions, including patient interactions and alternative plans.
• Collaborate with interdisciplinary teams to ensure seamless coordination of services and resources for patients.
• Offer situational counseling to address patients' emotional needs and facilitate adjustment to medical challenges.

Requirements:

• Minimum of 1 year of experience in discharge planning, care coordination, or case management.
• At least 1 year of experience as a Licensed Social Worker (LSW) in a hospital setting.
• Active Wisconsin Social Work License.
• Master of Social Work (MSW) degree required.

SALARY:

• Starting pay at: $50 / hour

Additional Details:

• Start: ASAP Contract: 13 weeks initially with extensions
• Hours: 8am-4:30pm
• Benefits: Medical, Dental, 401k, Referral bonus, Weekly pay
