Job Summary:
We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will provide quality patient care by assessing, planning, implementing, and evaluating patient needs. This role involves collaborating with healthcare providers and patients to ensure optimal outcomes.
Responsibilities:
- Conduct comprehensive patient assessments
- Develop and implement individualized care plans
- Monitor and adjust treatment plans as necessary
- Coordinate with healthcare providers to deliver integrated care
- Educate patients and families on health management strategies
- Maintain accurate medical records and documentation
- Ensure compliance with regulatory standards and protocols
Experience:
- Previous experience in critical care or diagnostic evaluation
- Strong physiology knowledge and proficiency in medical terminology
- Familiarity with managing medical records in a hospital setting
- Experience working with managed care organizations
- Exceptional patient service skills with a focus on vital signs monitoring
- Preferred experience in Pediatric Intensive Care Unit (PICU) settings
