Medical College of Wisconsin is Hiring a Registered Nurse Case Manager - Kenosha Near Kenosha, WI

Position Description:

Women's Intensive HIV Nurse Case Manager - Kenosha

Provides HIV nurse case management to youth (ages 17-25) and women living with HIV. Works collaboratively with multi-disciplinary care teams that may include clients and their support systems, practitioners, nurses, social workers, and other providers. Supports program requirements through data collection and timely documentation. Serves as a recognized expert in the care of people living with HIV and as a consultant to health care providers within Kenosha, Wisconsin and surrounding areas.

Responsibilities:

  • Responsible for providing intensive nurse case management of referred youth (ages 17-25) and women living with HIV. Complete intake, assessments and develop service plans. Provide care coordination, HIV education and prevention.
  • Complete home and community-based visits working extensively to support medication adherence, health literacy, and client engagement in medical care, while maintaining client confidentiality.
  • Collaborate with a diverse group of multidisciplinary care team members (physicians, nurses, social workers, community providers, etc.), to proactively plan and implement care and service plan goals. Monitor changes and respond to follow-up needs of clients, follow through on identified referrals in a timely manner, and communicate changes and needs with all members of a client's interdisciplinary care team.
  • Manage and maintain program data for enrolled clients. Acquire necessary patient information from off-site clinical venues and ensure accurate inclusion in the program database. Collaborate with the WI HIV Primary Care Support Network to ensure appropriate grant related data points are included in the Network data system.
  • Seek out and develop client education materials on a wide range of topics (HIV prevention, disease progression, medication adherence etc.), educate clients, their support systems, and members of the multidisciplinary team on treatment options for youth (ages 17-25) and women living with HIV.
  • Act as a resource, advocate, and serve as a care consultant within the community and healthcare system on behalf of the clients with, or at-risk for, HIV.
  • Maintain appropriate and up-to-date knowledge of HIV treatment options through reading professional journals, affiliation with appropriate professional or community organizations, attendance at interdisciplinary care team meetings, collaborative case conference meetings, and other professional opportunities.
  • Demonstrates proficiency in the use of the electronic medical record (EMR).
  • Participates in HIV Primary Care Support Network quality initiatives and activities.
  • Perform related duties as required or directed by the Ryan White Program Director.

Knowledge - Skills - Abilities

Knowledge of biology, clinical judgment, medicine, cultural awareness, and consulting. Excellent communication and organizational skills for supporting patients, patient support systems, physicians, and team members under stressful conditions. Should be detail oriented and able to accurately track complex data required for patients' longitudinal care. Reliability and ability to protect client confidentiality a must. Compassion and commitment to caring for people living with complex health needs and social disparities including: poverty, mental illness, drug use, LGBTQ issues, and other complex social barriers.

Preferred Schedule: Full-time role with expectations for coverage during core business hours and flexibility required as necessary to accommodate business needs.

Position Requirements:

Minimum Qualifications:Appropriate experience may be substituted for education on an equivalent basis

  • Bachelor's degree
  • 1 year


Preferred Qualifications:

  • Bachelor's degree in Nursing, Bilingual in Spanish and English
  • 2-5 years' health care experience in managing patients with chronic and complex health care needs. Motivational interviewing experience (professional certificate of completion required within the first year of hire).

Certification: Current Wisconsin RN License. Current CPR certification. Valid Wisconsin Driver's License.
